Question: i have f4 liver fibrosis and cirrhotic morphology of the liver - what is the prognosis for treatment and life expectancy? do i need a liver transplant? is this a death sentence?

1 I would like to know your liver function test report
2 Creatinine and sodium report
3 what is the geno type of hepatitis c
4 have you ever experienced swelling over feet or fluid in your abdomen
5 have you ever experienced abnormal behaviour or coma which was attributed to hepatitis c
6 are hepatitis b and HIV negative
7 what are hemoglobin and platelet values
8 are you on any other meds
I know it is a long list but most of this information is necessary to give a proper answers to the questions you have asked.

geno type hepatitis c is 1a
yes swelling over feet and no fluid in abdomen
no abnormal behavior or coma
only hep c - i do not have hep b or hiv all negative
platelets count as of 9/10/14 was 98
hemoglobin 16.3
only medications are diovan for blood pressure - i do not take anything else except standard multivitamins
creantinine,serum is 0.56
hepatitis c quantitation is 811,630
hcv log10 is 5.909
sodium,serum is 136
albumin,serum is 3.5
bilirubin,total is 1.5
ast is 131
alt is 158
lipid panel is all normal
AFP serum,tumor maker is 8.9
GGT is 74
all cardiovascular tests are normal

Do not need liver transplant

Detailed Answer:
Hello, You have advanced fibrosis and raised enzymes with a high vira load.
Your cirrhisis is well compensated.
You need treatment for hepatitis C.
The 10 year survival is to the tune of 76 percent. In case the liver disease becomes decompensated - jaundice, fluid in abdomen or coma develops the it falls down to 50 % at 5 years.
You do not seem to be a candidate for liver transplant at present.
You can consider treatment with pegylated interferron with ribavarin. Th treatment response rated are around 30 - 50 percent . High fibrosis scores and high viral load are indicators of lower response rates.
In case Sofosbuvir is available in the area where you live then it is a good option as the response rates in genotype on approach 90 Percent
