What Does This Ultrasound Scan Report Indicate?

Question: Went in for ultrasound scan for follicle growth measurements on cd 10. Over 10+ follicles seen, however the 3 largest measured 12mm, 10mm, and 0.9.mm. Took clomid 150 mgs for 5 days starting cd 4. Going back in for another scan tomorrow, which will be cd 12 only. They are trying to see how they are growing for when I can be given the hcg trigger shot and schedule the IUI procedure. 

I always have a regular period each month and this is my 2nd month being on clomid. I canceled last month's IUI due to losing my job and not being prepared financially for pregnancy. I have snice then gotten a new job and will be moving forward with the IUI this cycle. I am worried that something is wrong with me because the follicles never grow this slowly for me so far. And last month, since I took the Clomid but didn't end up taking the hcg trigger shot, I must have ovulated on my own. But my period was late and didn't start until cd 33. Do you think this clomid is messing me up and do you think that by tomorrow, the follicles will be higher in measurement so that I can so the IUI sometime soon and take that shot?

I am so so nervous and all I do is worry now about these follicle sizes as this has never been a problem for me in the past and by cd 12 they were already telling me to take the hcg trigger shot, but things are progressing much slower this month and I am scared that I hurt myself or something? :(

I understand your anxiety about the issue and I'm an infertility specialist myself so I understand this anxiety even better.
Clomiphene is a wonderful drug and thus don't keep anything against the drug per se
The growth of follicle is definitely a little slow but there is nothing to worry as it can be escalated by FSH injections in case your doctor feels like it.
You may have to wait for another 2-3 days for HCG trigger and that is the only thing apparent right now.
It is common to have delayed period after clomid so nothing to worry.

I have seen the attached report and glad to inform that 15 mm is a good size follicle and usual rate of growth of follicle is 2 mm per day so once your follicle reaches 18-20 mm she will give you trigger and thus good chances of pregnancy.
Don't keep any negative thoughts in your mind and start believing that you will be pregnant soon.
Faith is everything at times and can do miracles.

thank you for your encouragement. based on that report, how many eggs do you think will be released in a few days when I actually do the hcg shot? looks like a lot of them are already at 11mm. what are your thoughts on how many follicles will be released at induced ovulation from the hcg shot?

At least 3-4 follicles after 2 days

At least one follicle is needed as you know but if we delay the shot by 2 days then there are chances of at least 3-4 follicles if adequate maturity and one follicle of full maturity.

so, you think there is a possibility that 4 eggs could be fertilized? if they are only 11mm today, then it 2 days they will be around 15-16. isn't that too small to be considered mature? once they are released from the hcg shot, do they still have time to mature as I am aware that the egg only lasts for 24 hrs. I will probably end up taking the hcg shot 2 days from now, so I am just wondering what the others ones would have to be around for them to be fertilized as well as the lead one at 15mm right now. do you think it is ok to wait a bit longer to do the trigger shot or does it have to be given as soon as one follicle reaches 18mm?

So no need to wait longer.
Yes egg stays viable 24 hours only so IUI is times accordingly.

ok...promise this is my last question!!!

how big do you think/expect the smaller ones that are at 11mm right now, to get, if I am giving the hcg shot in 2 days? Monday or tuesday? I would like for there to be more than just 1 viable/mature egg to be able to be fertilized...just to increase my odds!

also, what is your opinion on miscarriage? do you think that bad/defective sperm could be the cause for a miscarriage or does it always 100% haveto be a problem with the egg?

You are most welcome dear.
Yes there are more chances of one very good sized and mature egg and at least 2 eggs of adequate maturity to be released after a shot on Monday and this number may increase to 4 if shot is given on Tuesday.
But the decision on the day of shot will stay with your gynecologist as she will (even I do) focus more on the biggest follicle which is the best and the dominant follicle.
IUI is a very good procedure and has very nice success rates so will give added advantage.
Quality of egg and sperm both matter for the success of conception and plays role in both implantation and pregnancy.
But since quality of sperm can be easily studied but quality of egg can only be indirectly anticipated so we can give various supplements to improve the quality of sperm prior to conception so as improve the quality of embryo.
