What Does This Ultrasound Report Of Ovaries Indicate?

Question: I am on my first round of Femara started day 3-7 and had an ultrasound on CD 11 and was told I had no follicles.. Then on CD 12 I got a positive OPK... I am confused whether or not this month has a chance for pregnancy since I had no follicles on CD 11, but had a positive opk on CD 12?

Thanks for the query to hcm,
This is indeed a confusing situation but an ultrasound is direct viewing of the ovaries and if no follicles are seen ( which is again unlikely on CD11) with a positive OPK on CD12 indicating that LH surge has occurred and ovulation should take place in next 36 hrs.
In my opinion you should get a transvaginal ultrasound pelvis which has a higher sensitivity as compared to an abdominal ultrasound. An LH SURGE will not occur in absence of follicular growth or in absence of an estrogenised environment.
You should repeat an ultrasound from a different radiologist and recheck for LH SURGE gain after 24 hrs with a different ovation kit. That should help clearing the picture.

I missed to mention that I did have a vaginal ultrasound for the CD11 when I was told I had no follicles appear.

I have now called back and they said they would preform another vaginal on Monday which I will then be CD 14.

YES, that would be the best way , to get a repeat vaginal ultrasound on CD14 to check for crenated or ruptured follicles if at all.
A repeat ultrasound shall clear the situation.

Also I was told apparantly there was no follicles on either side and that I probably did not respond to the Femara...

If they do the ultrasound on Monday and my positive OPK is to be true. What should they expect to see in the ultrasound on Monday since I will be CD 14 and received my positive OPK on CD12?

There should be Unruptured follicles and ruptured sac of follicle which should be seen on ultrasound,as the OPK test is positive.
If there are no follicles on vaginal ultrasound at all, then i suggest get a FSH and LH levels done and AMH count.
