What Does This Ultrasound Report For TB Indicate?

Question: Hello Doctor,
My sister is diagnosed with micobacterium tuberculosis
TB Antigen NIL is 194.08
UltraSound Report:
Rt. Ovary: "A hypoechoic solid complex mass irregular in shape and size 133x76*120mm appear to arise from right fallopian tube
Also XXXXXXX test is not good XXXXXXX (premenopausal) 10.47 XXXXXXX (Postmenopausal) 29.110
CA125 @ 60.7
RMI is not yet done.
Will it need to be operated? And is it completely curable?
Medication for TB is already going on.
our doctor is suggested for RMI on 19th of this month and she is telling that we need to operate it definitely( may be this month)

Now let us discuss the problems here.
THe first is tuberculosis ( TB ).
Does she have any symptoms of respiratory TB ?
WHy was this blood test done ?
Now the second issue of the ovarian mass.
IT is quite a big sized one, and yes, surgery is definitely indicated.
THE XXXXXXX score is borderline, and at her age ( 25 ), malignancy ( cancer ) is not very likely, but still RMI should be calculated.
Also, such a big tumour can be completely removed only by surgery, and it is essential to remove it and send it for biopsy so that all doubts regarding cancer can be settled.
Please ensure that the surgery is carried out only by a qualified and XXXXXXX / reputed Onco - Gynecologist, not by a routine gynecologist, since the surgery should be very detailed, thorough and should remove all doubts regarding cancer, and the tumour should be completely removed.
IT is curable completely if surgery is properly done, and treatment is taken as per the biopsy reports.
Is your sister married ?
RMI is also an essential finding, so please upload that report to me too once it is done.
But to clarify your doubts, yes, such a big mass needs surgical removal ( operation ) and yes, it is mostly curable to a great extent.

No, there is no sign of respiratory TB.
Not sure, why blood test is done. Will get the information and update you.
Is this XXXXXXX test is also a concern. What should we do about it?

Also, she is in XXXXXXX Should we go for the surgery in XXXXXXX PGI or Can you suggest some specialist in XXXXXXX itself.
Also, will surgery be done only after curing the TB, or it can be done anytime.

IT is borderline, so one cannot comment based on results.
RMI will give more information.
Right now, you can do nothing about it, but just wait for the RMI.
See, I am not aware of any onco gynec centres that are good in XXXXXXX
I was in PGI XXXXXXX for 6 years, and unfortunately, I cannot recommend anybody very good there also.
I would suggest XXXXXXX XXXXXXX Cancer Hospital in XXXXXXX or Tata Hospital in XXXXXXX or GCRI in XXXXXXX
Those are the places I personally know that are very good, otherwise I cannot comment on other places.
TB treatment will take atleast 6 months, you cannot wait until then for this big mass.
So surgery will have to be done before treatment for TB is completed.

Could you please suggest any doctor there.

Should I go ahead and book the appointment?
Also, Previously I have mentioned about RMI test which is pending. Actually, it not RMI test but MRI test which doctor has suggested. Is it fine? What is the difference?
How much time whole procedure will take? How long will we need to stay in Delhi?
Actually I am working in XXXXXXX I can call them here also if any good hospital is there. Please suggest.
One more thing, How much expense I should expect?

Hello again.
Sorry for a delayed response, as I had no internet access over the weekend.
I am not personally acquainted with Dr. XXXXXXX Rawal, but yes, he is a big name and has a very good reputation, so you will be fine with him.
You can book the appointment.
I have personally been familiar with GCRI XXXXXXX XXXXXXX so can suggest a few names there, rest , you have to go with the website.
ALso, RMI is ultrasound score to calculate likelihood of malignancy, so MRI will be done to assess that.
It is fine.
THe difference is that MRI is the technique used to calculate the RMI, which is a score.
I do not know how much time the whole procedure will take, since it involves getting an appointment, then consultation, further tests, actual surgery and post op period.
Typically, she will need 15 days rest after the surgery before travelling again.
COst again depends on the hospital, and type of surgery - open, laparoscopic etc.
SO I cannot give you an exact idea . XXXXXXX has excellent centres - HCG hospital, XXXXXXX has a wing called Mazumdar - XXXXXXX cancer centre. XXXXXXX is quite a good option too, if convenient for you.
