Question: I had a kind of dizzy feeling which may come and go at times. this problem started two years ago. I did all master health check ups(Heart check, Endoscopy, CT Scan and complete health check) to rule out any major problems, and all the reports were absolutely normal. Where in the physician suggested me to go for an ENT check.
I had the Ent check now, I have uploaded the reports.

Brief Answer:
Vestibular rehabilitation exercises

Detailed Answer:
Hi,

Thank you for your query.

1. You have labyrinthine paralysis on the left as per the reports.

2. The cause is usually a labyrinthitis or diabetic neuropathy and in many cases the cause is unknown.

3. Usually after 6-8 weeks there is cerebral compensation. Your dizziness has been there for 2 years, hence the cause and the amount of compensation is difficult to detect.

4. Vestibular rehabilitation exercises will help.

Hope I have answered your query. If you have any follow up queries, I will be available to answer them.

Regards.

Dear Doctor,
I would like to know more detail about the problem diagnosed.
Is there anything serious to look at?
The doctor had prescribed these medicines for the next two months.
1) Pirapil - 1200 mg (Piracetam) twice daily
2) Bilovas - 40 mg ( Ginkgo Biloba)
Will these medicines help me get a cure. Are the above medications safe for my medical profile.

About Me:
I am 32 year XXXXXXX
165 cms 93 kgs
Non Vegetarian
Smoking ( 10 per day)
Alcohol ( Heavy twice a week in parties, but quit past four months)
Medications from five years:
1) Inderal 10 mg for mild blood pressure ( Reducing the dose , its normal now)
2) Restyl 0.5 mg when ever needed for anxiety related issue
Recent medications
1)Cinitapride 3 mg for gastric trouble prescribed for two months.
2)Bupropion 150 mg sr from two weeks as prescribed by psychiatrist for anxiety and depression related issue.
I would like to know if the existing medications could have any negative interaction with the new ENT medications. Are there any side Effects.
Can i take all the medicines, is it safe.
As the doctor was new to me and was in a hurry, i was not able to ask my doubts clearly.
I hope i am able to explain my problem to you.
Please suggest.

Brief Answer:
Avoid Bupropion, Inderal, Restyl and Cinitapride.

Detailed Answer:
Hi,

Thank you for writing back.

1. The current ENT medication consisting of piracetam and ginkgo biloba is safe . however it is not a cure. Since the cause is unknown, treatment of the hypoactive labyrinth is difficult. It may recover on its own or the brain will compensate.

2. Buroprion is most likely to increase dizziness along with propranolol (Inderal) if your blood pressure is now within normal limits. Try and omit these four medications (Inderal, Restyl, Cinitapride and Bupropion) as soon as possible.

3. Keep a regular schedule of Vestibular Rehabilitation Exercises.

Hope I have answered your query. If you have any follow up queries, I will be available to answer them.

Regards.

Dear Doctor,

1)The cardiologist had suggested me not to stop inderal suddenly, and suggested me to reduce the dose gradually and then stop.

2)I have some anxiety and sleep problems , so i would find difficult without restyl.
I will stop Bupropion and Cinitapride.
3) How do i find the cause of my problem, Is this a major or serious problem.
4) What kind of Vestibular Rehabilitation Exercises should be done.

Brief Answer:
Inderal is low dose. Get VRE hand-out.

Detailed Answer:
Hi,

Thank you for writing back.

1. The Inderal dose of 10 mg is a fairly low dose compared to the usual initial 40 mg once or twice a day which is raised as required. Hence the tapering period at your dose is very short, if any.

2. You may continue the Restyl, however it is a habit forming drug.

3. Locating the cause after two years have elapsed is very difficult. It cannot be a serious cause since it has lasted two years already.

4. Get a patient hand-out of Vestibular Rehabilitation Exercises from a nearby ENT OPD.

Hope I have answered your query. If you have any follow up queries, I will be available to answer them.

Regards.

Thanks for reply .
As i mentioned the problem is from 2 years, i would like to say more clearly that the symptoms are more and noticeable from 6 months.
I am really worried, I would like to ask a few more doubts,
1) Is my problem of the ear related any brain damage
2) Is my problem of the ear due to excessive binge alcohol consumption
3) I have done a CT Scan which appeared to be normal. Is there any need to do a MRI Scan.
4) Please suggest any Online link of Vestibular Rehabilitation Exercises.

Brief Answer:
Not related to brain damage.

Detailed Answer:
Hi,

Thank you for writing back.

1. The labyrinthine paralysis is a peripheral receptor damage. It is not related to any brain damage. The brain will compensate with the help of VRE.

2. This is a good time to stop binge drinking. Binge drinking can lead to damage to the entire body. Damage to the auditory pathway is a relatively mild side effect.

3. An MRI will always provide more comprehensive information in addition to a CT Scan. If you do want to go ahead, get at least a 3 Tesla resolution.

4. A simple online search for VRE will suffice.

Hope I have answered your query. If you have any follow up queries, I will be available to answer them.
