What Does The Following MRI Report Indicate?

Question: Hi,
How are you?
I have just recently taken MRI scan for my hand. (Both My LEFT FOREARM and LEFT WRIST)
Could you explain to me what it means, how is the progress of my condition compared to previous condition (ECU tendonitis)
Brief Medical History: I injured my left wrist before, then i was diagnosed ECU tendonitis. I had not taken any MRI scan since then until recently. (since 2013)

MRI LEFT FOREARM:
Technique:
Sagittal, coronal, axial PD and T2 sequences were obtained.

Findings:
There is no bone marrow oedema, fracture or dislocation.
The muscles show normal striated appearance and return normal signal.
There is no soft tissue mass or muscle atrophy.
The neurovascular bundles appear unremarkable.
There is normal alignment of the elbow joint.

Conclusion:
No abnormality is seen in MRI of the forearm

MRI LEFT WRIST

Technique:
Thin section coronal, axial and sagittal, PD and P2 sequences were obtained.
Findings:
Neutral ulnar variance is noted.
There is dorsal subluxation of the distal radioulnar joint (DRUJ), that presumes disruption of the dorsal radioulnar ligament.
There is focal bone bruise in the lunate bone. No fracture line is identified. The rest of the bones return normal signal.
There is focal thickening with increased signal in the extensor carpi ulnaris tendon in keeping with severe tendonitis. The rest of the tendons are intact and return normal signal.
The ligaments around the wrist joint are intact specifically the scapho-lunate ligament and the TFCC.
There is no soft tissue mass or muscle atrophy. The median nerve appears normal.
Conclusion:
There is dorsal subluxation of the distal radioulnar joint (DRUJ), that presumes disruption of the dorsal radioulnar ligament.
There is focal bone bruise in the lunate bone, without any fracture line.
There is focal thickening with increased signal in the extensor carpi ulnaris tendon in keeping with severe tendonitis.

You have mild dislocation- subluxation of radio ulnar joint at the wrist due to ligament injury- Can be due to inflammation in the area.
The inflammation of tendon carpi ulnaris is noted. The bone bruise can also be due to a chronic inflammation.
You need to give adequate rest to the wrist and take anti inflammatory medicines. In case there is no improvement with proper treatment then surgical repair of the ligament and subluxation can be considered.

Hello.
Thanks for your reply.
Could you explain to me what would have caused for the dislocation-subluxation of my radio ulnar joint (wrist) ? as before i was only told that i only have ECU tendonitis hence i did quite lots of exercises.. strengthening wrist with dumbell, stretching exercises, swimming etc (both fingers and tendons). i did not expect for my ligament to be hurt. and from the report, it says that it shows focal thickening with increased signal in the ECU tendons.keeping with severe tendonitis. -->
can you explain to me why i still have ECU tendonitis... because i had this for quite sometime,,, and i thought that my tendons had already healed after sometime. if its not, how to help the tendons to heal faster? i supposed i could not do exercises again as i would give the wrist a rest... and the tendons(ECU), fingers are not fully recovered, still abit weak, i still shake or drop when i carry, hold small things.

Can be due to stress related injury.

Hello.
Thanks for writing again.
The subluxation is due to ligament injury caused by the stress during exercise.
The tendonitis has not healed and is still persistent. You need to give rest to the wrist by using a brace. Avoid all painful movements. Local hot compress thrice a day and TENS therapy can help in early recovery.
Local steroid injections can decrease the inflammation for few weeks.
The focal bone bruise is related to an injury to the area- it can be a stress related injury.

Again, thanks for your reply.
Sorry i need to ask you few more questions.
Can you give me guidelines for the proper treatment on what kind of treatment, and for which part first and the duration.
What should i start first?


i took celebrex before, maybe that can help me again to reduce the inflammation for both my ligament(subluxation) and tendonitis... and i will also ask for local steroid injections. and for how many weeks????can the inflammation subside.

Celebrex and hot compress will help.

Detailed Answer:
The treatment in your case should include oral anti inflammatory medicines - Celebrex can help. You need to use a brace to give proper rest to the arm. Local steroid injections can help. The duration of treatment will depend upon the response to the treatment.
After the acute pain subsides you need to start with physiotherapy exercises under the guidance of a proficient physiotherapist. In case no improvement is noted even after 6 weeks of doing all this then a surgical repair should be considered.
