What Does The Follicular Study Indicate About My Chances Of Getting Pregnant?

Question: Hello Doctor,

I am 31 years old and trying to conceive for the last 6 -8 months. I was diagnosed with PCOD in 2011 and after getting treatment for 1 year, PCOD was cured as per two TVS scans in 2012. I do have frequent urine infections and had undergone antibiotic courses twice last year and one this year in 2014.
Since I am trying to conceive, my Gynae has prescribed UBIPHENE 100 mg from Day 3 of menses for 7 days; Progynova from Day 4 of menses for 10 days and Duphaston Tablet from Day 16 for the last two months. I am also taking OVACET 3 times a day for this month and folic acid supplement. This month doctor put me in a Follicular study on D-12, D-14 & D-16. I had 2 follicles on my left ovaries measuring 26x17mm and 27x22mm and 9X8mm follicle in my right ovary. The follicles grew to a size of 34X18mm and 34X21mm (LO) on Day 14 and 10X7mm on RO. On day 16th, it measured 35.7X23mm and 37.7 X20.4mm on my LO and 10.7x7mm on my right ovary. Since I could not show my reports right after the first two scans, Doc advised later that we shall try to rupture the follicle in the next cycle and reduced the dosage of Ubiphene from 100 mg to 50mg as I was having a good follicle growth on Day 12th itself. My endometrial thickness measured 6.8 mm on D 12th and increased to 8.6mm on Day 16th. Although I had PCOD a couple of years back, I always had regular periods and currently have 30-32 day cycle every month. I believe my follicle did not rupture on time and hence need to go for another study and take steps to rupture the follicle in my next cycle. My Progesterone Level was 4.17 ng/ml on Day 16 of my cycle and Prolactin Level 17 ng/ml.

What does the above suggest? Any problems or am I not ovulating on time or any issues with ovulation etc. I am desperately trying to be pregnant and hence seek your advice. Kindly advice.
Also note that I just stopped taking Duphaston after 15 days on 31st May and today is the 3rd of XXXXXXX Any chances that I might be pregnant or I need to wait and check for few more days.

I read your query and I understand your concerns.

Following is my reply:

1) PCOD cannot be cured just like that. There must be something wrong in the scan first time.

2) Follicle if not ruptured you need to get Inj. hCG 5000 IU from your doctor to rupture when follicle reaches 18mm size.

3) The chance of pregnancy is there. Please get serum beta hCG blood test to confirm the pregnancy if you dont get cycle in another 3 days.

Could you also confirm if I am ovulating as the follicle did not rupture till day 16. Progesterone Level on Day 16 has shown 4.17ng/ml. Is there a chance that I am ovulating late. Also, since the follicles in my right ovary outgrew the normal 18-mm/21mm; is there a chance that would have got ruptured after Day 16 as I was having main in my pelvic region; cramps to be precise and a shooting cramp at times with breast tenderness and nipple soreness.

Hi,

As you had pain after day 16, it is possible that you have ovulated little late. But it is difficult to say now. Progesterone is low though.

If you dont conceive this cycle, It is better to do urine test to confirm ovulation next time along with scan. You can use strip called i-sure which confirms ovulation along with scan.

Hi Doc,

Please elaborate on the PCOS thing you mentioned above. What did you imply when you said that it cannot disappear and the sacn must be incorrect.

When diagonised with PCOS in 2011, I had done three TVS scans and on medication there was improvement on the condition. On my 4th and 5th scan in 2012, Doctor said Ovaries are functioning normally as per the scan report.

Please advice.

Hi,

There are certain criteria to diagnose PCOS:

1) The follicle in ovary must be more than 10 each side.

2) Or you must have delayed cycles or no cycles every month.

3) Or there must be signs of hyperandrogenemia like facial hair etc..

If you were diagnosed as PCOS based on any of these criteria it is not really possible to have done away with it in just one year. Hence there is something lacking in information. Wrong diagnosis is always a possibility.

Sure, I would set up an appointment at your clinic after checking my condition this week if I get my menses or else advised I will check for Pregnancy Test.

on PCOS,

1. I always had regular periods except when there is a cycle change every 6-7 months, periods would be delayed by 1-2 weeks
2. I did not have hyperandrogenemia
3. PCOS was only diagnosed based on TVS scan when I complained of pelvic region pain/cramp and pain during intercourse
The scan report said multiple cysts in both ovaries. That was way back in 2011. All these latest reports of scan suggest only 1-2 follicles in each ovary and hence no symptom of PCOD.

If the PCOS was diagnosed on scan to have multiple cyst in both ovaries, it cannot suddenly reduce to 1 or 2 follicles with i 2 to 3 years.
