Question: HELLO DOCTOR, i have swelling in he right side neck, and i showed to ENT, and Dr. has prescribed some tests, MY BLOOD TEST ( TB GOLD QUANTIFERON ) RESULTS : QUANTIFERON IS POSITIVE AND INF y LEVELS 3.41 MG, IS IT A TB, What it exactly means, was it a TB, of what stage, how to treat, Dr. had given me AKT-4, i enclose reports also with this, please guide me.

Brief Answer: Tuberculosis , continue treatment. Detailed Answer: Hi XXXXXXX After reviewing your reports. I confirm that you have tuberculosis of cervical lymph nodes. As you must be aware of tuberculosis is an infection by a bacteria called Mycobacterium tuberculosis. It can affect any parts of the body. For you it has affected the lymph node. Well it is difficult to find out what stage you are in. The Tb gold quantiferon is serology test to detect TB Antibody titre and it's significantly high. This indicates you have this infection. Your doctor may order for a lymph node biopsy from one of the swollen node to confirm the infection (if necessary). As far as I am concerned, you can get other tests like ESR, mantoux test, Sputum analysis, PCR for TB done to confirm your diagnosis. In the meanwhile continue AKT4 kit for almost 3 to 6 months depending on your clinical symptoms. Continue other medicines as prescribed. Don't worry disease is treatable. Hope this helps. dear madam, i am satisfied with the reply, i want to know further info, and want to upload some further reports and info about.. In dec 2013 i got swelling and pain in the right ear and swelling at right side of neck, i showed to ent, and the dr, prescribed some tests and state some operation had to be done for sinus from nose... after that i have used tablets dr has prescribed, and stopped after one month. later in the end of feb and in the beaning of feb, i found some more swelling in the same side of neck and showed to other doctor,, which i had given you earlier.. and it states that it was TB,, so i request you to see both the reports and advice me what to do.. both are different or same... and other issue is i am taking ATK4 as prescribed by dr, for TB, i am getting seviour headache, drowsiness, weakness,, and lot of body pains.. are they the side effects of the Atk4 , what additional should i take.. please also see the new attachment uploaded by me

Brief Answer: parotid infection, sinusitis, TB neck. Detailed Answer: Hi XXXXXXX I went through your reports and the previous doctor prescription. According to his diagnosis, your ear pain and swelling in front of the ear was because of acute infection of your parotid gland. He has confirmed this by ultrasound and CT scan. He has treated you for that parotid infection. In the CT scan, in the nose your central bone of nose is deviated to right side with little changes in your maxillary sinus suggestive of sinusitis. In your history you have never complained about frequent cold, nose block nasal discharge. May be you had it at that time, so they have treated you accordingly. But, seeing your CT scan report, in my opinion I dont think you need surgery for sinuses. Your sinus infection can be treated with medicines itself. If it keeps recurring then you can go for surgery. Next report is related to your TB lymph nodes of neck. CT scan also shows tubercular changes in the lymph node like calcification. The diagnosis, investigation, ,treatment is correct. Continue that. As you said your headache, drowsiness weakness is because of the side effects of AKT4. You have been already prescribef with multivitamin for that. Continue that. Along with this., in my opinion other blood test like Liver function test .renal function tests will be done twice a week, after a starting treatment to check for toxicity, If the test reports suggest toxicity then, dosage will be reduced or certain drugs are stopped, So for this please consult your doctor.
