What Does My Ultrasound Scan Report Indicate?

Question: I had a miscarriage in Aug and now I'm pregnant again. My ultrasound read the following. Does that mean it's going to miscarry again?


IMPRESSION:

1. Single live intrauterine gestation at approximately 6 weeks and 1
day. There has been interval growth of the gestational sac since the
prior study.

2. Persistent bradycardia with intermittent periods of no cardiac
activity. These findings raise concern for a poor prognostic outcome
of the pregnancy. A follow-up ultrasound is suggested in 2-3 weeks to
determine viability.


FINDINGS:


Ultrasound examination of the gravid uterus was performed
transabdominally and transvaginally in the Ultrasound Section.
Transvaginal imaging was done for better visualization of the
endometrium.

The uterus is anteverted in position and contains an intrauterine
gestational sac with a mean sac diameter of 1.8 cm which corresponds
to a current gestational age of 6 weeks and 6 days. There is an
embryo identified with a crown rump length of 0.41 cm, which
corresponds to a current gestational age of 6 weeks and 1 day.
Cardiac activity is identified at a rate of 66 bpm beats per minute.
The cardiac activity was intermittent with periods of no cardiac
activity. A yolk sac is identified.



The right ovary measures 2.0 cm x 2.0 cm x 2.4 cm. The left ovary
measures 2.7 cm x 2.6 cm x 4.3 cm.



There is no abnormal free fluid identified in the cul-de-sac.

Coming to the present ultrasound scan findings, a pregnancy in its earliest stages is present in the uterus. This is the normal location for a pregnancy.

At this stage the age of the pregnancy is calculated using the diameter of the gestation sac and the crown rump length CRL of the embryo.

The gestational sac diameter for your pregnancy is 1.8 cm and this matches with the age of the embryo as 6 weeks 6 days.

The CRL of the embryo measures 0.41 cm and this shows the growing fetus is 6 weeks 1 day.

When we have the gestation sac and CRL measurements, the CRL age is followed and therefore the report mentions the age as 6 weeks 1 day.

The heart of the embryo starts beating around the 5th to 6th weeks and your pregnancy shows the embryo with a cardiac activity of 66 beats per minute. However this heart beat is not consistent and is showing gaps when the heart is not beating. This can happen either because the heart is still in its earliest stages of development or rarely, it can be due to a developmentally weak heart.

The yolk sac provides all the nutrients the embryo needs and produces blood cells until the placenta fully forms later in the pregnancy.

It is important to do the scan again after 2-3 weeks duration to say that heart beats have picked up and are consistent in rhythm and showing normal embryonic growth. This is to confirm the normal growth of the embryo for the rest of the pregnancy.
