What Does My Sperm Analysis Test Report Indicate?

Question: Hello Doctor,
I'm 27 and my wife is 22 years old. Attached my sperm analysis and culture test report. My doctor says that motility is a little on lower side of normal range. Although he didn't sound very confident about a natural pregnancy. The semen culture report showed some infection. I also underwent a scrotal scan and he has found some infection outside. He has prescribed me Tomoxifen (10mg) - 30 days, Doxycyclin(100mg) - 10 days * 2 times a day, Levoflox(500mg) - 14 days * 2 times a day , and Paternia XT - 30 days * 2 times a day and has asked me to repeat the sperm analysis again after a month. We've been married for 1.5 years although we've just started trying for a baby. Is my report normal? Is there any chance of natural pregnancy or should we explore other options like IUI etc.,?

1. Sperm should have a normal motility to enable good chances of pregnancy. Asthenospermia is a condition in which there is reduced motility of the sperms and this reduces the chances of successful pregnancy. You have this mild condition and this might be treated as recommended by your doctor.

2. Your medicines are the following
(i) Tamoxifen is a medicine prescribed for asthenospermia.
(ii) Doxycyclin is an antibiotic to treat infections.
(iii) Levofloxacin is an antibiotic to treat infections
(iv) Paternia is a combination of micronutrients containing Co – Q10, L- Carnitine, Lycopene and Zinc; these help in increasing sperm motility.

3. Your report shows that the sperm motility is below normal and in many patients, this might be corrected by taking medicines to treat any infection and take supplements which enhance sperm motility.

4. I have come across patients who have normal report after taking treatment for few months and your report also might come back normal after treatment with above medicines.

5. Natural pregnancy chances are still there and only if there is no positive result after treatment then you should go for assisted reproduction techniques. There is no need to panic and hopefully your reports will come back normal after on month.

Hello Doctor,

Thanks for you response. During this one month, is there any lifestyle changes or food intake changes that could be made to increase motility? I'd been under antidepressant Prodep-20MG and Trapex-1MG. The dose was tapered and stopped last month. Do these have any adverse effects on motility? We'd done semen analysis on XXXXXXX 2015. I didn't look through the report much carefully but according to our doctor the report was normal. Also, in my current report attached the sperm concentration is 39mil/ML against a normal value of 15 mil/ML. I'm just curious if in any way the lower motility might be offset by higher concentration.

1. The medicine Prodep contains fluoxetine and taking this might reduce sperm motility significantly. Probably that is why your sperm motility is reuced.

2. Trapex is Lorazepam and it might have little influence on fertility.

3. The normal sperm count is any thing above 15 million per ml. Your count is 30 million per ml and is normal. Therefore the argument on higher concentration from lower motility does not exist

Is tamoxifen a regularly prescribed medicine for this condition? Is it safe to take this drug for 30 days (once a day)? What kind of side effects should I look out for?

1. The medicine tamoxifen is regularly recommended for short duration use to improve chances of pregnancy. Tamoxifen increases serum follicle stimulating hormone and testosterone and this helps in increasing fertility.

2. Research shows that taking the medicine for 30 days does not cause any significant serious adverse effects.

3. Side effects like loss of libido, tiredness, sleepiness or a feeling of lethargy are seen when used for more than a year of treatment in other conditions.

Thank you very much for your patience. Ever since the semen analysis results, I'm under severe psychological stress. I'm unable to have deep sleep and have heart palpitations during day time. I'm planning to go back to my psychiatrist. Mostly I would be asked to continue my previous medication of Prodep and Trapex 1MG. Is is OK to take both tamoxifen and fluoxetine together. I'm worried of any adverse side effects. Or is there any safer alternative that can be taken with tamoxifen instead of fluoxetine.

1. I understand your anxiety and stress levels. Please take a short break from your work and get your mind relaxed. The stress is affecting your sperm quality and the medicines are also having a side effect.

2. In my opinion please try and join any yoga center and spend doing meditation for 30 minutes every day.

3. Continuation of Prodep and Trapex is on recommendation of your psychiatrist. It is difficult to know your level of stress without clinical examination and your psychiatrist might have evaluated you in detail. You might have a discussion with them and tell about the decreased sperm motility and then try and stop the medicine if you find mental relaxation by doing yoga and meditation.
