What Does My Sperm Analysis Report Indicate?

Question: Hello,

I am XXXXXXX and married for 3.5 years.we having been trying for a baby for 2 years.I have a 2 cm cyst in right ovary and my husband has bilateral grade 2 varicocele. I have regular cycles (26 days).my husband's normal morphology is 2%, 20 mill/ml sperm count and 45% motility and has been on acupuncture treatment and vitamins for 4 months as we did not want to go for varicocele surgery. But there has been no significant improvement and have been advised for fertility treatment.

One iui had failed in November 2016.

Please advise us on the best fertility treatments that we would need to undergo with high chance of conception. Also, let us know if natural conception is possible with the present sperm quality.
Also, do I need to take a second opinion with an XXXXXXX

I think you should go for evaluation first. Do a thyroid profile and prolactin level for yourself if not done already.

For your husband, I think he can take second opinion from an XXXXXXX Also do a semen culture and sensitivity test. There can be some seminal infections. He can take some antibiotics like doxycycline 100 mg twice a day for 10 days empirically. Get a CASA done. It is a computer based semen analysis. Also go for a ROS and DFI. If these are on a higher side, you may need to undergo IVF with ICSI.

Natural conception is possible as sperm count and motility is normal. But there is high chance of miscarriages with such poor morph. In my opinion, you can go for a varicocele operation and see the effect on semen parameters.

You can try 3 to 5 more cycles of IUI. The chance to get pregnant in one cycle of IUI is 10 to 15 percent only. So, you can try atleast 6 cycles of IUI.
