Hi and thank you for asking on HCM!
I read carefully all your concerns and reports and I can say as follows :
1. With regards to transaminases (AST and AlT)level. Yes it is true that they are elevated at your case.
The main cause of elevated transaminases is viral hepatitis and autoimmune disorders.
At your case you have rule out other issues like ceruloplasmine (normal).
As I read above you are waiting the test reports of viral hepatitis and autoimmune panel results.
2.You have high pro thrombine time level. It is probably related with warfarine use.
So it doesn't indicate a hepatic issues.
3. I would like to know the opinion of you hematologist about high Hemoglobin level?I would like to know too if you have used alcohol and how much per week.
At the end I need to know the last results for viral hepatitis and autoimmune hepatitis to give an accurate opinion.

Thank you for your response. I have included the results of the Hepatitis and Autoimmunnity Studies that were not available when I submitted my initial inquiry:

1. See below Autoimmunity Studies and Hepatitis Testing. I'm also including the year long results of my ALT and AST. You will note at the time I presented at the ER my AST and ALT were both normal. My Alkaline Phosphatase has always been well within the normal averaging 52 unit/L (Reference Range: 32-92 unit/L)

2. Due to warfarin use my Prothrombin Time (PT) over the last year has ranged from a low of 13.6 to a high of 25.3

3. My Hematologist has not provided any insight since the Jak 2 Exon 12 Mutation Dx test was negative. She referred me to a Heptologist that ordered the additional labs I've shared with you. I don't know, yet, what is causing my Hemoglobin and Hematocrit to be elevated. Both were normal until April 29 labs. The July 22, labs was the first time my Estimated GFR, Non XXXXXXX XXXXXXX was low at 47 mL/min/1.73m2 (Low). Previous nine eGFR results were all >70 with the exception of a 66. My alcohol consumption is sporatic. I'm a social drinker that's more likely to over indulge than drink regularly. Most weeks I have <10 beers. More on vacation and social occassions.


I had a CT and MRI last year. One interpretation observed an enlarged an enlarged spleen, some fat deposition and suggested Portal Hypertension.

Hi back,
I read carefully all your medical reports and I can say as follows:
1. Your serological viral hepatitis tests are negativ . It shows that your transaminasw level are not associated with viral hepatitis infection.
The only one that I didn't found is EBV and CMV infection. Which I would recommend you to do .
2. autoimune hepatitis has been ruled out from your test results.
This is why to my opinion your Ast and Alt level may be related with :
- medication ( please tell me if you have started any treatment in the same time or before the transaminase level increased)
- EBV or CMV acute infection.
3. With regards to Ct report please can you upload it to me so I can check them by myself.

Thank you for your time and attention to my questions.

I have uploaded the radiology reports as you requested.

I have not been tested for CMV and EBV. I had a very bad case of mono almost 40 years when I was in college. I am due to have more labs drawn later this month. I will request labs for CMV and EBV.

The only medications I'm taking that I was not taking a year ago are warfarin, metoprolol (August 2015-January 2016) and Cardizem LA 120mg (January 2016-Present). I developed side effects from metoprolol as the reason for the change in afib medication.

Hi back,
I read carefully your Ct and MRI reports and it shows about an acute portal vein thrombosis.
If I were your treating doctor I would recommend to repeat abdominal ultrasound or Ct now at to better evaluate If the thrombus is still there and if there is any structural damage of liver.
Otherwise it is important that you have started a new medication like cardizem. It has rarely( nearly 2% of cases ) side effects like elevation of AST and AlT level. So it can be a factor of your elevated transaminases.
But we should wait till you finish all your exams and get a final conclusion.
