Brief Answer:
Get him tested for typhoid.

Detailed Answer:
Thanks for asking on HealthcareMagic.

I have gone very carefully through your query. Let me assure you that for an infant (like your child), the normal WBC count can vary between 5000 and 25000 and hence 20300 is not something to worry about. But an ongoing fever of 4 weeks is definitely a cause for concern.

You need to get the blood tested for C-Reactive protein (CRP) levels and widal test for typhoid. Continue to give paracetamol to bring down the temperature when it rises.

It is very important to give semi-solid food to your child at this age. Milk would not be adequate and that could make his defence mechanisms weak thereby making him more prone to infections.

Not even cerelac or steamed apple or banana or suji or XXXXXXX ..nothing he eats....n even if some bits go into his mouth he vomits everything even what milk he had 2-3hrs back....concerned abt this also...does he have throat infection? Plus everyday he just opens his mouth to vomit but smtimes he doesn't maybe he only feels like vomiting.pls help.

Brief Answer:
Milk alone cannot meet the energy requirements of your child.

Detailed Answer:
Thanks for writing back. I understand what you say. But this cannot go on forever. You need to take your child to a doctor. The doctor would be able to guide you elaborately on what is to be done and what not to be done. You might be doing the things in a wrong way and some simple steps could resolve the problem.

The first step would be to give him liquids like dal water, fruit juices. The next step would be to introduce mashed boiled potatoes and mashed bananas.

thank u doctor, just one more clarification, as you hv mentioned that the WBC count is usually high in infants, there is also 22% of abnormal cells (atypical lymphocytes) showing in the report, is that also normal or smthg to worry? and since he is small the test is done through finger prick, does this give accurate results? i have mentioned to his pediatrician abt his diet, but he said its ok as of now and nothing to worry.thanks.

Brief Answer:
Microscopic evaluation of blood smear is essential.

Detailed Answer:
Thanks for writing back. The finger prick helps in the microscopic look up of the smear and it is fairly adequate. I would not really be bothered with the diet.
