What Does My Antenatal Scan Report Indicate?

Question: Hello Doctor,

We had a antenatal scan and the report says the below:

Single intra -uterine live fetus seen in Cephalic presentation at the time of scan

Placenta: position posterior upper segment
Maturity : Grade -|||
Liquor : more (AFI=22-24cm) Deepest pocket =9cm, smallest pocket = 6 cm
cervix : 3.6 cm

Average Gestation age : 33 weeks 3 days

Fetal movements are normal
Fetal Heart rate is regular - 130 beats /min
Fetal Weight : 2162 +/- 316g

Impression:
Polyhydramnios

Mild Subcutaneous increased transculency noted around nape of neck(6-7mm)..? significant
Left uterine and umbilical arteries show borderline increased resistance and S/D ratios... For follow up

Could you please let us know if there any complications in the report and please advise what are the necessary actions to be taken if any.

Also, about AFI index its showing 22-24 cm , is it a normal and please let us know if any risks on having that much index.

I am Dr.Srilatha gynaecologist.
I have gone through your query.

Following are the ultrasound report findings:-
Polyhydramnios means presence of excess liquor.

It might be due to congenital anomalies of fetus,placental abnormalities,Diabetes in mother etc.
As per your report there are no placental abnormalities.

There is mild translucency noted around nape of neck. It might be not that significant at this gestational age.

This finding is more significant in early pregnancy.
Excess liquor is associated with preterm labor,preterm rupture of membranes,cord prolapse etc.
But these complications might not be there with all the cases of excess liquor.

So do not worry.
Increased resistance and S/D ratio suggest mild intrauterine growth retardation.
It needs frequent monitoring to check for the well being of fetus.

Take high protein diet.

Avoid physical strain and take good rest.

Diabetes in mother should be ruled out by blood sugars.

Hello Doctor,

Thank you much for your reply and suggestions.

Have question on 'Increased resistance and S/D ratio suggest mild intrauterine growth retardation'.
Could you please let me know what leads to Increased resistance and S/D ratio.
Please let me know what precautions to be taken to avoid high resistance and improve the growth normal.
Now at 33 weeks 3 days fetal is weighing 2162 +/- 316g , is that ok or any precautions to be taken care.

Proper surveillance of fetus is needed

Hi there,good day.
I got your query.
The increased umbilical artery and uterine artery resistance and S/D ratio suggests placental insufficiency.

Placental insufficiency means there will be decreased blood flow to fetus due to increased resistance in the umbilical and uterine arteries.

There are many causes for placental insufficiency like :-
1)Diabetes ,hypertension,anaemia,any substance abuse like smoking ,maternal anxiety etc.
2)Placental causes like clots or inforcts in blood vessels.
3)Congenital anamolies of foetus.

This placental insufficiency might affect the growth of the fetus.

So I would like you to check for any high BP, diabetes and anaemia and get treatment accordingly.

Avoid stress or anxiety as it is also one of the cause.

Yes the fetal weight is adequate for the 33 weeks.

Bed rest, high protein diet and drugs like aspirin might be helpful.

Aspirin decreases the clot formation and cause dilatation of blood vessels.

So discuss regarding that with your doctor.
Also consider to rule out the above said causes of placental insufficiency.
