Question: My brother is 42-he went for his physical and his blood was drawn. His numbers are are not adding up--I don't have the exact numbers, I'm trying to get a hold of the report. His bilirubin is very high, his platelets are very low, his red and white blood cells are normal and his liver enzymes are normal?!?!?!! He is normal weight, doesn't drink alcohol and eats healthy. I think he needs to be referred to a Hematologist. What can be happening to him?

High bilirubin level may indicate liver disease. It is important to know which part of bilirubin is increased, means direct or indirect. This is important in diagnosing etiology. Ultrasound of abdomen can be done.
Higher bilirubin levels may indicate increased rate of destruction of red blood cells(hemolysis).
Low platelet level may be due to many causes like infection etc.He may need platelet infusion. This need proper assessment. This condition need proper clinical examination and further investigations.
