Question: I am 34 year old, my height is 6 ft 4 inch. I weigh 118 kg. I had bought an insurance product for which i was subject to a medical test. The urine and blood report has come normal. The cholestrol levels were also within prescribed limits. However the ECG showed a Bifid p wave. Which is why they have refused to give me critical illness rider on my policy. Request you to tell me the significance of this and what medical or dietary steps should I be taking to address this at personal level.

Detailed Answer: Dear Sir The T wave inversion in lead AvL is ~1mm. Any T wave inversion less than 2 mm is not considered relevant unless there is a strong clinical correlation which is not there in your case. The bifid P waves are visible in lead II, III and V2-4. However they do not meet the criteria of significance for "left atrial abnormality' which states that either the P wave duration should be more than 110 ms or distance between two humps of bifid P should be more than 40 ms; both of them are not there in this ECG. Still lead V1 shows negative P waves which meet the criteria of significance for Left atrial abnormality (product of duration and height/ voltage appear more than 0.03 mm sec). All this may be a normal variant, but I would recommend you to consult your cardiologist and get examined. If he agrees, next step is a simple ECHO which we will clarify everything. Your blood reports are all within normal limits except for serum triglyceride which is marginally high but it does not require any drug therapy. Try to do exercise daily. Avoid excess of saturated fats and sugars. Maintain ideal body weight. These things will help to bring it to normal. Hope this helps. Feel free to discuss further. Sincerely Sukhvinder
