What Do White Patches On The Finger Tip Suggest?

Question: Hi Doctor,

I have observed white patches on my wife's (Age:26) finger tips from last week. I have consulted Dermatologist however she has suggested to perform some tests and came to know that vitamin D is very low which is less that 4.

will this be curable?

She seems to be having vitiligo

Hi.

I have reviewed the Images and reports. I can see de-pigmented patches on her digits. She seems to have Vitiligo.
She has been asked to apply sorfil-s by her doctir which is calcipotriol and betamethasone dipropionate cream. I suggest that she continues with it.
Vitiligo is a slow responding disease and treatment may last months. It is advisable to follow up with your doctor once every 1-2 months and if required, for any modification to treatment if the patches don't respond Or increase in number Or size.

Hi.

Yes, she can get pregnant.
Her low vitamin D levels have to supplemented from outside e.g she can take a XXXXXXX of calcirol 60000 IU once every week. Kindly check with your doctor if he/ she has already prescribed a Vitamin D supplement to correct her vitamin d levels.

Hi Kakkar,

I am attaching all the reports (File name: Final Report) which i have taken as suggest by doctor and also including the Medicines currently my wife is using.

Current Medication: Seacod (Tab) daily one), Thyronom (12.5mcg) (half tablet a day), Meex 12 (morning one tab), Nuvijen- CT (Night one tab), Lycopex -Q10 (3 times a day), UPA -Plus (2 times a day), Suncee+ (3 times a day) and EX D3 one XXXXXXX (1 gram) (Mon - wed-Fri -Sun)

And also applying Sorfil -s ointment.

She didn't use any tabs or any thing in past we have started all these from this week only.

Please look into it and kindly suggest your opinion.

Hi.

I have reviewed the reports. Apart from low Vitamin D levels and Raised TSH levels (hypothyroidism) rest of her report seems normal.
She has already been asked to supplement her low Vit D levels (EX D3 sachet).
Besides antioxidants like lycopene (Lycopex-Q 10) and Vitamin C (Suncee +) may play a beneficial role in vitiligo patients. She can continue.
Sorfil- S ointment is a combination of calcipotriol and betamethasone dipropionate in an ointment base. These are a treatment modality in vitiligo.
Thyroid abnomalitis are more common in vitiligo patients and may be related to autoimmune nature of both these conditions. Thyroid abnormalities need to be corrected appropriately, just as in her case.
So, all in all, I agree with this initial treatment. How her skin conditions responds to this treatment remains to be seen.
Vitiligo is a slow responding disease. That is why your doctor has asked you to follow after 2 months. However, if new lesions appear or existing lesions spread you may report early to tour doctor for appropriate adjustments to treatment.
Other treatment modalities in vitiligo include topical steroids, tacrolimus ointment, basic- fibroblast growth factor, pseudocatalase, targeted Broadband phototherapy, oral steroids, psoralens+UVA, PUVAsol etc.
A dermatologist can choose and prescribe one or more out of these modalities depending on the progress and response to treatment.
