default
Posted on Mon, 29 Dec 2014
Question: Hi,
My serum Vitamin D (25 hydroxy Vitamin D) is 12.5 ng/ml.
Alkaline phosphatase (SAP) is 222 u/l.
Vitamin B12 (Cyanocobalamin) is 328 pg/ml.
HbA1c is 8.5.

There is consistent pain in waist region and leg, difficulty in using stairs or sitting on floor and standing back. Can not walk for longer distances (say for 3-4 km) as it makes leg pain severe.

Does above point to Osteomalacia and Osteoporosis and based on above symptoms and readings, how far am i from any serious problem?
How soon should i consult Orthopedic specialist or other suitable specialist as suggested by you?

Brief Answer:
Suggest mri

Detailed Answer:
Hi, I am Dr Mittal. I have read your query. I think I would be able to help you. The reports are suggestive of
a. Low vitamin d
B. Onset of diabetes or uncontrolled diabetes.
C. Back problem with possible nerve compression causing lower leg radiating pain.

I think I would suggest that you have a test - Mri of the spine of the back of the area where you have the pain. I suspect that the mri will show some compression on the nerves along with stiffness of the muscles that may occur due to bone and muscle weakness secondary to vitamin d deficiency and made more prominent due to uncontrolled diabetes.

Till that time, you can meet up with a doctor for an additional prescription of pregabalin.

I have tried to make it as simple as possible. I hope that the information helps you. Please feel free to contact us for more information. Best of luck. Dr Mittal.
