Posted on Mon, 11 Jan 2016
Question: hello doctor, i used to be a drinker i quit drinking on 7 august 2015 and i had my liver function test on 11 september 2015 and i got ELEVATED ALT = 108 which should be less than 50 and AST = 42 & GGTP = 43 earlier consulting doctor here they told me it will get normal within a month and in between i did course of UDILIV 300mg for 14 days twice a day and today almost after 3 months and 7 days i again had my liver function test and kidney function test and my ALT level is still elevated that is 93, ALT = 93 and AST = 36 & GGTP = 25 , in 3 months i do agree that my AST & GGTP level get more lower in figures but my ALT level is still elevated to 93 which should be less than 50, i also went for ultrasound in which i got normal liver , i am also attaching my today's report so that you can check it.

i am really looking forward for healthy life and also its been 4 months to me i never drink alcohol and in between i eat plenty of fruits also drink lemonade(s) so that my ALT level get lower than 50 unit but its still high so i want to know how can i decrease my ALT level that is below 50.

Brief Answer:
SGPT 93 is considered as a clinically normal.

Detailed Answer:
Hello,
Welcome to HCM.

I have seen the attached investigation report here and can understand your concern about SGPT.

Here your SGPT (93) will be considered as a clinically normal despite it is marginally elevated (depending upon the lab variations) and it would have no clinical significance in an otherwise healthy individual.Rest of other liver and kidney function tests reports are completely normal.

I would not be personally worried about it, since SGPT (ALT) is highly sensitive liver function test and it might get easily elevated even with fatty meal or even mild alcohol intake.

The other causes of SGPT rise could be use of pain killers (NSAIDs),use of various types of non herbal health supplements, obesity and heavy exercises or some autoimmune disease or liver disorders like hepatitis B or C but they are very unlikely in your case.

Your ultrasound abdomen is also normal which is indicative of there is no underling pathology of liver or any liver damage.

So, do not worry about it and possibly avoid fatty/junk/oily foods and alcohol which you have already stopped.

However, you can repeat the test for SGPT after 4 weeks on fasting blood sample and most probably it will come within normal limits.

Meanwhile, you may continue to take tab.Udilive 300 mg twice daily which is a bile acid, a substance naturally produced by the body and does not have any side effects.

You may also take syrup LIV-52 DS 2TSF daily which is a herbal preparation having hepato-protective property and may be beneficial for you.

You can also check you liver markers for hepatitis b and lipid profile and if this both will come negative or normal then I do not think there is anything to be concerned in your case.

Hope this helps and let me know if you have a more questions.

ok thank you but i want to know how many days i should take UDILIV 300mg for? and i hope there is no side effects of taking UDILIV 300mg as i am little bit concerned of these allopathic medicines because i have read on wikipedia internet that UDILIV too has side effects on liver that sometimes it leads to cirrhosis.

and earlier also i took udiliv 300 mg and test came normal and whenever i stop taking UDILIV 300mg my ALT level comes elevated. Also i want to tell that i used to be very heavy drinker very heavy drinker 4 months ago .

also i want to ask you one more thing recently a week ago i got into fight with someone and guy pressed my throat and he rolled his fingers on my throat with a pressure & pressing for about 25 to 30 seconds.. my concern in this is now i am feeling pain on my right side only upper part of throat on right side while swallowing food and drinking water but on my neck there is no swelling neither there is any bruise or any clott. but its paining while swallowing food and drinking water so why i am now having pain on my upper right side of throat after a week also

Will answer one by one.

Detailed Answer:
Hello,
Welcome again,

1) As i said earlier,Udilive 300 mg is a bile acid, a substance naturally produced by the body and does not have any side effects or very occasionally it may cause nausea, vomiting, headache, dizzy but these side effects are infrequent and not generally seen even on long term usage of udiliv.

In my advice, you should continue udilive for more 4 weeks and avoid all kinds of fatty/oily foods or pain killers if you take.

However,it might take plenty of months (say 6 months) to come down your SGPT level to normal in blood since you were a heavy drinker early on.

Also,SGPT is a highly sensitive liver function tests and may get easily elevated even with mild alcohol intake or fatty food as I already discussed.

So,no need to be worried much as it is clinically normal and most probably does not indicative of any liver disease.

2) Pain during swallowing could be due to neck muscle sprain or stretching occurred when someone has pressed your neck with his fingers or hands on throat.

You may take OTC pain killer (diclofenac 100) if there is unbearable pain during swallowing and it may take up to 7 to 10 days to get rid off this.

Since there is no swelling or bruise then no need to be worry about it or in case,if symptoms lasts longer even after 10 days then consult ENT specialist for throat examination with pharyngoscopy.

Hope this helps and let me know if you need more clarifications.

ok i also want to ask you if i want to celebrate can i have alcohol now or i should not have alcohol till my LFT screening SGPT level comes to normal .

and how much drink it would be safe for me to have right now if i want to celebrate christmas and new year and also for future.

i want to know drinking alcohol elevates which liver enzymes ALT AST or GGTP most.

and if i want to have bacardi breezer then would it safe for me to have 1 according to my LFT report.

and after taking and completing UDILIV course for 4 weeks then when i should again go for my LFT screening because finishing course and going next day only my report for obvious reason will come normal only as i was on medication therefore after completing the course when should i go again for my lft screening.

and i am having pain right side above on throat that also while moving tongue and swallowing and its a bearable pain only.

and also i want to know whats this A:G ratio means in this LFT screening because my A:G ratio is slightly above that is 2.10 and it should be between 0.9 - 2.00

i mainly want to know how i should drink alcohol so that my AST ALT and GGTP levels dont elevate it always be in balance that is below 50.

and as you know my AST is 93 right now so i want to know whether while having alcohol occasionaly only on weekends will going to make that increase in future or decrease and also once it gets decrease and i drink alcohol occasionalt will there is chance of getting my these GGTP SGPT and AST levels high elevated in future

thank you.

Detailed Answer:
Hello,
Welcome again,

I have already clarified regarding your marginally elevated SGPT (93) which does not have any clinical significance, so I would not be personally worried about this findings and so you should too.

However, even mild alcohol intake (say 2 or 3 pegs) might cause rise in liver enzymes SGPT which is highly sensitive.

So,it would be better to complete avoid it if you are much bother about SGPT level.

But there will be no issue to drink 2 or 3 pegs occasionally on Christmas eve which will not cause any liver damage but yes,it might cause rise (although mild) in liver enzyme SGPT up to a certain extent (may be up to 125.

But as we discussed, it is clinically not abnormal and it may come back to normal within 2 to 4 weeks.

So,you may continue to take udilive 300 mg for next 4 weeks and after that you can go for retesting of SGPT which may come normal or if it will come slightly high (upto 100) then there is also no need to worry.

A:G ration 2.10 indicates proportion of albumin protein to globulin protein in blood and which is almost normal and not a worrisome finding.

For,throat or neck pain you may take symptommatic OTC medications which we have already discussed.

Let me know if anything is unclear.
