What Do These Basic Health Checkup Reports Indicate?

Question: Hi , some time back, I had asked you suggestions for basic health checkup. Dr. Bhagyalaxmi Nalaparaju suggested me to take checkup package offered by Thyrocare. I have the reports of the test. Request you to please help interpret the reports and advise on specific issues.

Regarding XXXXX's reports, the reports are mostly normal and are to be correlated with symptoms clinically. Vit D is low and should be supplemented with a dose of atleast 20,000 IU weekly. Apart from this there is nothing much t worry about.

Regarding XXXXXX's reports, the findings are suggestive of some bacterial infection, low vit D, some liver issues, high uric acid. For low vit D, supplements in the same dose as mentioned above are to be taken. For liver issues, abstain from drinking alcohol. Drink water from RO system purification. For high uric acid nothing needs to be done if there are no joint pains. If joint swelling and pain is present then see an orthopedic surgeon for it.

I have the following follow-up questions

1) Vitamin D deficiency: Which supplements should we take? Do I need a prescription for it?
2) Bacterial infection: We have a RO system at home and even at office I consume bottled water only. I usually don't have any bowl movement issues. Should I get any additional tests done?
3) Uric acid: I had consulted an Orthopedic specialist about 6 months back after severe pain near big toe of the left foot (no inflammation or redness). The suggested Uric acid test confirmed high levels and the doctor prescribed Febutaz (one pill daily for 60 days; which I continued). Since then, I don't have any pain but should I continue the medication? Also, can some dietary changes/addition help in the long term. Ideally, I would not want to consume any medicine.

5) High LDL cholesterol: I am a little concerned about LDL cholesterol (131 against 85-130 normal range). I exercise regularly (4-5 days a week, vigorous Cardio and strength for about a couple of years now; I am 5' 5'' and weigh 68 kg) and my diet is majorly vegetarian; subzi + roti for lunch on all days and subzi+roti+daal+rice for dinner on all days. On an average, I consume, non-vegetarian food (mostly chicken or eggs) only in 1 or 2 meals in a week. Alcohol too, is limited to once a week. I quit smoking about 9 months back (no relapse since). My parents (60+) and grandparents (80+) don't have any known heart conditions. Are there any dietary or lifestyle changes suggested? Should I follow up with a cholesterol test after 3-6 months?

6) Low Total Thoraxine levels: I would also request you to comment on the low Total Thoraxine levels (3.6 vs. 4.5-12.0 normal range). I believe, low levels result into low metabolism. Can something be done about this only through dietary changes/inclusions or exercise?

7) High Testosterone levels for my wife: Madhuri's Testosterone levels are 104.27 vs. 14-76 normal level for adult females. She is 30 years old. We are planning to get pregnant in 6 months. Do you think this is a matter of concern? Please advise.

Medicine to be based on symptoms

Detailed Answer:
I will answer your queries specifically.

1. You should also get tested for serum calcium levels. If Calcium is also low, then supplements like keycal XXXXXXX can be taken. If only vit D is low then supplements like calcikind XXXXXXX be taken. Calcikind has 60000 IU. This much dose can be taken weekly for a month and then biweekly for 6 months. Retest in 6 months.

2. No need for further tests for bacterial infection if there are no symptoms of infection.

3. Regarding uric acid I personally think episodic treatment can be taken if joint pain happens. Alcohol has to be avoided at all costs. Then foods rich in purines are to be avoided like palak paneer, mushroom, meats.

5. I am not worried about your LDL levels. Still continue your low fat diet and the levels will not get worse. Avoid high calorie foods like milk products (cheese, butter, ghee, sweets, etc). Include more of fruits and salads in your diet. Exercise daily for 20 min will help a lot. May followup for lipid profile in 1 - 2 years.

6. Re test thyroid profile in 3 - 6 months. TSH is normal so most probably it is not affecting your metabolism. Diet and exercise will not help.

7. High testosterone in females causes unwanted facial and body hair, hoarsening of voice etc. If these symptoms are not present then dont worry. Isolated high testosterone levels do not affect fertility. Anyways there must be a source of high testosterone. We can get her tested for PCOS by getting ultrasound of pelvis done. Even if some cause is found, there is no need for special precautions or medicines for fertility unless you have regularly tried for one year and still unable to conceive.
