What Do The Following Test Reports Indicate?

Question: Hello Doctor,


      We are trying for baby since 2years. Last year we consulted an gynecologist and an XXXXXXX for the same. 
      They suggested ICSI as I am 40 yr old and my husband is 44 year old.
This is second marriage for both of us.I don't have any child from my first marriage and my Husband have two kids (20 yr old daughter and 18 yr old son) from his first marriage.
      My husband's kidney was transplanted in 1991 i.e approx. 24 yr back and he is on the following medication since then :-
1) Ecosprin 150.             1-0-0
2) Wyasolone 7.5 mg        1-0-0
3) cellcept 500 mg          1-0-1
4) Neoral 50 mg             1-0-1
5) Arkamin H.                1-0-1
6) Tenormin 50.              1-0-1
7) lasix.                       0-0-1
8) Zyloric 100 mg.           1-0-1
9) Methycobal.               1-0-1
10) Sevkar 400.              1-1-1
11) shelcal OS.               0-1-0

      My husband's reports showed the following readings before we underwent ICSI
1) F.S.H.                     4.15
2) L.H.                       6.22
3) Total Testosterone     557.19
4) Aldosterone              13.2

The semen Analysis observed the following value:-
1) volume.                     0.8 ml
2) sperm concentration.     00
3) total sperm count         00

With these reports doctor suggested some test:-
1) ULTRASOUND-TRUS-PROSTATE AND SEMINAL VESICLES 
2) M.R.I OF PELVIS"
All test done were normal.

On the basis of my husband's history of Renal transplant,our age and his above mentioned reports doctor suggested to do testicular sperm aspiration for ICSI.
 
Multiple  needle biopsies on both testes did not reveal presence of any Spermatozoa so my oocytes were vitrified.
 After that my husband was put on an Ayurvedic capsules called SPERMON (2-2) for 3 months and I was on OVAFEM PLUS once a day.

After 3months of above medication we again did the following test and the readings are as follows:-
1) F.S.H.                    17.83
2) L.H.                      33.06
3) Prolactin.               13.35
4) Total Testosterone    963.19
5) Free Testosterone.    11.00
7) Progesterone.          0.3
8) SHBG.                   32.6
9) Oestradiol              94

With all these readings high my husband stopped the medicine. And later then after 2months we once again did the following test 
1) Oestradiol.             52
2) Total Testosterone.   557.19

The semen Analysis observed the following value:-
1) volume.                     1.00 ml
2) sperm concentration.     Single sperm seen in centrifuged deposit

Pls advice us as now which way to go.

I have read you query and I understand your concerns about your husband's sperm count.

Following is my reply:

1) His sperm counts are very low and are difficult to improve for a successful pregnancy.

2) His FSH levels are high indicationg there is not high chances of finding good sperms.

3) He needs to go ahead with donor sperms for fertilizing your ovum as chances of finding healthy sperms in future even with medication are low.

Hello doctor,
Thnx for ur reply.
Sry but we are nt ready for donor sperm
Can any medicine or stem Cell treatment will help us.
Doctor suggested the following medicine for my husband :-
1) Siphene
2) Maxoza L For 4 months
Later they said they will do micro dissection of testes as they are expecting few sperms with this medicines
Also we are ready to travel any part of the world for the same.

Stem cell is not yet operational. Micro TESA can be tried as an option which is last resort.

Let him continue the medicines he is taking. They are good.

Hello doctor,
Thnx for ur reply.

Doctor how long it will take approx for stem cell treatment to be operational because I think that's my last hope.
Also are the above medicine have 100%result n in wat time I can get the benefit , if nt can u suggest some result oriented medicine
Pls doctor advice me as I'm going thro a very depressed phase
1) Any medicine
2) Any treatment
3) Any where in the world if any treatment is available pls let me know
Time is also running out, but I can still wait if anything is there for my problem
I know I'm repeating the same question but I'm very much depressed
Doctor do you think that we have missed anything in the above issue...
I hope I have conveyed my problem

I have heard about stem cell treatment centers in XXXXXXX
1) Mother cell Regenerative centre pvt ltd
2) XXXXXXX Research centre
Can u suggest me about there treatment. Should I go with it? Do they have answer for my problem?

I understand your concerns about having husband's sperms only.

No medicine can improve his sperms 100 percent accurately.

Unfortunately stem cells to regenerate sperms are still in research stage in the world. No progress has been made yet and don't know when will it be made for clinical application. I dont think they do this in XXXXXXX
