What Do The Following Blood Test Results Of An Infant Indicate?

Question: Hi Doctor,
I am blessed with a bay girl recently.
There was a routine checkup for her yesterday in which the doctor suggested serum bilirubin test for jaundice. Report is attached.

1. Please check the report and advise if everything is ok?
2. Her nails have grown a little. shall we cut the nails or not at this early stage? If yes, how shall we cut them (using normal nail cutter?)
3. Her tongue seems a little dirty and left over of milk is seen on the tongue. How shall we clean it?
4. Her lips are a little bit dry. Shall we apply something?
5. Her face has developed some acnes. Pic attached. Please suggest if something has to be done or avoided.

Nothing to worry - bilirubin raise is physiological

Detailed Answer:
Hi...I understand your concern. I have gone through both the report and the image.

Coming to your queries -

1. The raise in bilirubin is physiological. For a 7 day old baby, it is normal, don't worry.

2. You can cut the nails with a baby nail cutter available in baby shops.

3. No need to clean the tongue, but at the same time please show it to your pediatrician and let him rule out oral thrush. But if the baby is feeding normally it is not oral thrush and you need not worry about.

4. For dry lips you can apply mother's milk itself. After she completes feeding in one breast, mildly squeeze the breast and apply the milk which is coming out. This milk will contain more amount of fat & it will take care.

5. This is not acne, this is infantile Eczema. The trick of the management is keeping the skin moist and never allowing it to be dry. For this oil massage and lotions like Oilatum will help.

Regarding cure options - eczema is a sort of allergy...allergies can only be controlled and can never be cured. But usually kids grow out of it...that is as the age progresses, they become better starting from 6 months.

Hi Doctor,
Thanks again for your reply.

Again, I have come back with few more queries regarding my baby:

1. While taking mother's feed, she (baby) generally sleeps or cries and not taking properly. If at all she takes, it is hardly for 5-10 minutes per try and then we feed her by extracting mother's milk in bottle and top up with external milk (Nan pro 1).
Is it ok to give external milk immediately after mother's feed?

2. How much mother's milk is sufficient for baby in a day as we are worried that she is not taking it properly.

3. Also, she (baby) sleeps sometimes for 3-4 hours continuously and then cries for milk. We generally don;t wake her up to feed her. She usually takes 60-90 ml per feed. Is it ok to feed her when she cries after sleep or shall we feed her every 2 hours by waking her up from sleep?

Coming to your queries -

1. You can give top feeding or formula feeding even after giving breast feeding, immediately also (provided the baby is able to accept it).

2. On average the baby can feed about 600ml per day of mother's milk.

3. I suggest you wake her up 2 hours once to feed the baby.

Hi Doctor,
Thanks for your reply. Sorry to bother you with so many queries but, this is my first baby so a little bit worried about the happenings.

1. Today we have noticed some yellow sticky substance (kind of discharge) in her diaper and toilet area. What it could be and is it something to worry about?

2. She is also making a little bit noise in breathing. Can/Shall we do something about it?
