Question: Hi Doctor,

Immigration doctor has asked below:

Requested test/report:
Chest clinic investigation on current state of tuberculosis

Could you please explain?

For this you need to consult pulmonologist and get done clinical examination of respiratory system and evaluation of x ray and CT scan by pulmonologist.
If he feels that all these are sufficient to stamp that you are not having active tuberculosis then no need for bronchoscopy and XXXXXXX analysis.
But if feels that bronchoscopy is needed then you have to get it done.

What is the meaning of CLINICAL EXAMINATION OF RESPIRATORY SYSTEM?

Please explain.

They have given me only 4 days to submit the report so i won't be able to go for any Sputum culture test as it takes approx. 2 months to get the results.

This is mostly stethoscope examination of your lung sounds, air entry in lungs etc.
Naked eye examination of chest by doctor is also included in clinical examination.

Hi Doctor,

Yesterday i went to see a pulmonologist and first he said to go for chest x-ray(PA) and when i submitted the report of this x-ray and having a look on the report then he said to go for another one which is apico-lordotic view of chest X-ray and i am still waiting on the report of 2nd one which i will get today.

Is the doctor supposed to follow this way?

Doctor said the report should be from a recognized lab by NZ immigration as previous x-ray which was carried out last month was not from a recognized lab.

New x-ray(PA) has below results:

1.Mild bilateral apical pleural thickening seen.

2. Bilateral increased lung vessels are seen.

3. No obvious active lung lesion visualized.

4. Costophrenic angles are clear.

5. Cardiac silhouette is clear.

6. Small right bony cervical rib is seen.

7. Soft tissues are normal.

After looking at below results doctor said to go for apicolordotic view chest x-ray.

X ray reporting similar to previous one.
No changes as compare to previous x ray.
But what about symptoms?
Do you have any symptoms like coughing, fever etc?
And yes apicolordotic x ray will be more informative. Actually this x ray is taken in such a way that apical lesions visualize better.
Since you are having apical thickening, apicolordotic view will be more informative.

I can understand your concern.
No need to worry if you are asymptomatic.
To answer about apical pleural thickening, I need to give some details first.
Lungs are surrounded by very thin pleural layers. They are not visible on chest x ray normally.
When lungs are insulted (either due to trauma or infection), surrounding pleural layers are also inflamed. With treatment lung will improve but pleural layers will heal by thickening. So this pleural thickening suggests old healed lesions.
Get done apicolordotic view.
In my opinion, you will get physical clearance after this apicolordotic view.
