What Do My CT Scan And Ultrasound Scan Test Reports Indicate?

default
Posted on Sun, 26 Jul 2015
Question: Hello,
I am seeking an opinion on the findings, reports attached, and also next course of action or treatment.
This is related to the condition of my Mom, who is 69 years old, and reported abdominal pain on the right, pale stools, and swelling in legs 2 weeks ago.
Pale stool symptoms and swelling in legs have reduced since then, but still having intermittent pain in the stomach along with bloating.

Please help explain the situation along with any guidance on the treatment plan. She has gone through Ultrasound scan and also CT-Scan. CT-Scan reports are attached below.

Thank you,
XXXX

Brief Answer:
confirmatory diagnosis

Detailed Answer:
Hi.
Thanks for your query and the reports done.
I am sorry that it looks to be a cancer of gallbladder. The reports are suggestive that the patient is most probably suffering from cancer of gall bladder that has spread to many parts and has a complication of obstructive jaundice.
I would advise you the following.
First of all to get a confirmation of the diagnosis.
A team of surgical and medical oncologist will work with your first to confirm the diagnosis. Once this is confirmed an appropriate package of treatment can be discussed with them. Hence choose a specialized centre for cancer.

I hope, this answers your query.

May I please know the opinion of Doctors you have consulted.

Please feel free to ask relevant query if you need to or if you feel that there is a gap of communication.

Thank you for the prompt reply.
Just a few lingering questions in mind about possible scenario.
Let us say biopsy confirms that be cancer:
- What stage would this be considered?
- What is the normal life expectancy at this stage?
- Will surgery be suggested as viable option to control the damage?
- Is surgery an option considering the age of my mom?

Brief Answer:
As explained

Detailed Answer:
Hi.
If this is proved by biopsy to be a cancer:
- Stage IV as distant metastases are present
- Life expectancy: every person responds differently. I am sorry this is difficult to say.
- Surgery is not the option at this stage,
- With better anesthesia and surgical instrumentation age is not a bar for any surgery.

Please give feedback of the Surgeon's at Gem Hospital...

Dear Dr. Chandrakant,

Thanks again for the reply.
This is what was recommended to her as treatment plan.

Visit 1 on Jun 26th:
A course of antibiotics(oral) for 15 days on the first visit, and also follow up CT-scan

Visit 2:
CT-scan was completed, you may have seen the attached report above, but Biopsy procedure is now recommended during this visit to collect blood samples from the liver - as described to them. Unfortunately, I did not accompany them during their visit to enquire more about the details.

Consulting doctor also discussed a possible treatment plan, during visit #2, of taking medications as injections once a day for next 15 days to clear the conditions that looks like mass/infections/clot if it is not malignant and no surgery is considered at this time - as described to them.

Would you concur with the treatment plan or any different ideas you may have?

Thank you much,
XXXX

Brief Answer:
Good plan - FNAC/ biopsy is already planned.

Detailed Answer:
To collect the samples:
This is called FNAC and good that if it done at the earliest.
A small time is taken as all the parameters of the blood, coagulation profile in particular are being checked.

This will give the definite diagnosis and a plan of management.
I think they have already planned for this as you have mentioned above.

Request for the diagnosis at the earliest, continue rest of the treatment as advised as is indicated.

Please let me know the results.
