What Could Cause The Waddling Issue While In Sedentry Recovery After A Hip Surgery Done Long After A Stem Cell Implant To Treat AMS?

Question: My neurologist suspects I have stiff person syndrome. I am 75. I understand this most commonly occurs in people under 40. Some of the symptoms of SPS are not exhibited by me: my spine is normal curvature whereas in a sps person the lumbar area is bent towards the belly button; no loss of fine motor skills; no muscle spasms anywhere yet I have had a waddling condition for the last year and a half. This started within a month of a hip replacement surgery done in XXXXXXX of 2017. I have had stem cell implant in 2012 due to AMS which is in complete remission. I did have prior to the hip surgery quite a few months (maybe for 6-7 months total off and on) of the couch and t.v. due to back issues so I suspect severe muscle atrophication which may be contributing to the waddeling issue.

Brief Answer:
SPS seems unlikely clinically.

Detailed Answer:

Hello,

I have read your question and understand your concerns.

Clinically, stiff person syndrome is characterized by muscle rigidity that waxes and wanes with concurrent spasms.

Usually, it begins in the axial muscles and extends to the proximal limb muscles, but the severity of the limb muscle involvement may overwhelm the axial muscle involvement (stiff limb syndrome).

So, if these symptoms are not met, SPS is unlikely.

Waddling issues, on the other hand, may be related to the deep sensation (or proprioception) impairment such the cases of spinal cord compression for example.

Muscles atrophy also may cause some waddling, but the compression of the posterior spinal cord should be excluded first.

Imaging of the spinal cord such MRI is necessary for this purpose.

Please discuss with your Doctor about these issues.

Hope you found the answer helpful.

Let me know if I can assist you further.

Take care.
