Question: Doctor I had sex 7 years ago and dance then I gave not. This year I started having chronic diarrhea I lost 20 pounds in 6 months. I went to my family doctor in Sept I got physical and everything came back good except slight anemia. Diagnosed with ibd. Well in November I got tired of it went to the emergency they did full panel on me blood tests and imaging and they found nothing except low in potassium and my lymph was at 20.5 as in sept it was 20.9. So it went down as I been stressing and having major anxiety. So I requested a hiv test but they told me they couldn't do it there I would have to see my doctor. So I went to a clinic did his rapid test came back negative. Then I went to my doctor last week and she sent me to lab corp for another his test. That came back negative as well. She also ran my cbc again and this time my lymph was 16.5 and over all 1.7 whatever that meant. She says I don't have hiv or aids but I continue to have symptoms like white spots on tongue that come off when I brush my teeth as I sleep with my mouth open. I also have these spots on my face I will enclose a spot on my nose and next to my nose. I also have petechia all over my body small red spots spread out on my body about 10 of them from my neck to legs. Also I have been bruising easy bruises out of no where. I also have a spot on the roof of mouth but my doctor said looked normal for some reason. I'm scared.

Since then I have not sorry had sex

Sorry in Sept lymph was 29 not 20.9

Brief Answer:
Further tests are warranted.

Detailed Answer:
Hi Mam,

I understand your query and concern and gone through your uploaded images. .

IBD is known to cause symptoms of immunodeficiency.

The white spots which you are referring to could probably be related to fungal infections.

The petechial spots all over your body require further investigations in the form of complete coagulation profile along with Ultrasound/CT of your liver, to document any liver pathology associated with IBD.

The lesion at the roof of the mouth requires biopsy to document its histology before coming to any conclusion.

Post your further queries, if any.
Thank you.

Brief Answer:
Complete ANA profile is needed.

Detailed Answer:
Hi Mam.

I am sorry not to mention about them in my first answer as I had trouble with my internet connection. I regret for the same.

The facial rash seems to be unrelated to HIV.

The rash seems to be autoimmune in origin related to IBD.

I advise you to get a complete panel of autoimmune antibodies screen-ANA profile.

Corticosteroids are the main stay of treatment to control this rash.

Regarding HIV,since you have been tested negative twice ,its highly unlikely to be HIV.

But the sure way of confirming it is through a series of tests in the sequence P24 capture assay followed by ELISA followed by WESTERN BLOT.

If you have not had the above sequence, i request you to undergo the same.If you have already got them, then kindly ignore.

Post your further queries,if any.
Thank you.
