Question: Hello

I saw the doctor. He said it's a rare diverticulum but probably it's not causing any symptoms, it's small and many other conditions mimic the problems I have

He also told me to forget a bit about this stuff

I find this complicated. I can't tell if the diverticulum is causing symptoms but I do feel problems on the location where it is
I also still wheeze and there's no answer from any doctor to that. I am extremely tired because I find no solution.

Was this Doctor an ENT or some other Specialist that you saw?

Please elaborate on wheezing, what exactly are the symptoms? Is it noise respiration or you have to take extra efforts to breath in and/or out?
Is there breathlessness on exertion? Your detailed history may give me clue to find its relationship with some problem as your Doctors are not finding the reasons of it.

Consulting a Pulmonologist, getting the Pulmonary Function tests can help for the proper diagnosis about wheezing.

Yes, it is true that the diverticulum that you have is very rare and usually may not cause symptoms but since you have symptoms in the same area as that of the diverticulum and also have bubbling or gurgling as you mentioned in the last questions, it may be taken as symptomatic.
I would advise you to take a second opinion of another Surgeon, may be a General Surgeon or Onco-Surgeon, the one who has experience in such a case.

It was a gastroenterologist

It's difficult to explain my wheezing. It's all in the right side of my throat.
The wheezing seems to come from my collarbone or probably above and around my cricopharynageal muscle.

There's usually phlegm like at the right side. It's really to explain if the phlegm comes from above the vocal folds or below. And it's always on the right side.
So I usually have to clear my throat tough. I try to not cause too much harm

The wheezing is like a whistling sound and it's present always. mostly on exhaling but it seems to come from upper airways but I don't seem to have real difficulty on breathing.. just the noise
I used to jog before but never felt shortness of breath there. I didn't go jogging for 2 years. At times I used to feel pressure on the right side of my throat and chest

There's no asthma but I had a spirometry, it revealed 101% but a lower airway reduction of 25%. My doctor said it was good and shouldn't woryr. However I cannot explain this well. Please let me know if you need more info.

I wished I could explain better as it's really difficult to explain
I have chest CT (2014 and 2016) a MRI of the head in 2012 and a neck CT scan on 2016.

I saw there's a radiologist on this website. Maybe he could take a look at them and find out things that maybe my radiologist missed?

I actually don't know which doctor to ask as my doctor said there might be nobody interested on doing surgery for me on this due to the risks..

I really wonder what else I can do. I really thank you for all your help..

Sincere thanks for your appreciation and feedback in details.
Read it carefully and understood that it is basically like around the pharynx, like pooling of saliva as I can understand from your description, that is the reason that you have to clear the throat.
Spirometry has been done and noted the findings that your have posted.
You should have started to jog, it helps therapeutically too.
Pressure on the throat on right side also noted along with the other explanations.
These symptoms may be correlated to the direticulum you have.

Surgical Gastroenterologist can tell you better hence try to get a second opinion as the Gastroenterologists' are from medicine background thus do not expertise as much as a Surgical Specialist .

The Radiologist needs all the films and usually they can see sequentially on their machines to correlate all the snaps in a sequence and hence possible to diagnosis any thing. Still films may not be as useful.

Yes, every Surgery has a risk involved but still have to take proper medication after the clinical evaluation and examination and its correlation the the present position.

The hearing of the whistling sound on open mouth indicate that you still have partial obstruction caused by the things you mentioned.

Please get an opinion of a Surgical Gastroenterologist and the one who has done such surgeries,

I really can't confirm isf it's really around the pharynx or under the larynx, very difficult to explain
Pooling of saliva could be, but I was seen by a laryngologist and it seemed to be clean. I have a video as well

I have the CDs of the scans so I assume they contain all the images. Should I send them?

I am trying to find surgeons, but as of now I don't seem to find anyone useful so I am also checking on other countries because I don't find them capable to do it (they weren't able to diagnose it either)

There can be vague symptoms in such a case - meaning one can not pin-point the area of symptoms. If there are no other diseases then obviously the one found on CT can be the reason for the symptoms in the neck and throat.

Hello

Thank you but seeing a surgeon here seems complicated. I will try to ask one. My gastroenterologist suggested not to have surgery.

However I am affraid, as damage to the recurrent laryngeal nerve would cause a vocal fold paralysis and would leave me with another problem, which is worst.

How can we check that the diverticulum is causing issues. How can we evaluate my swallowing and why do I wheeze. I wanna have surgery if it's indeed that the diverticulum is causing issues. Otherwise not.

2 neck CT scans of 2014 and 2015, folders "TAC Torax 2014" and "TAC Torax 2016" (on this one the diverticulum was found, but I suppose it's probably on the 2014's as well)
Neck CT scan with contrast located on "TAC Cuello 2016"

Please let me know if I should submit these to the radiologists on this website first or how to proceed.

No Surgeon would operate you unless he is sure about the relation between your symptoms and the radiological findings. So do not worry about these aspect. You can certainly say the same thing to your Surgeon.
Please do not be afraid of the recurrent laryngeal nerve as this is always to be found for every thyroid surgery and many at the neck region. This is routine for a Surgeon experienced in head and neck surgery, including that of cancers.

The Surgeon who will examine you would definitely correlate with these.
Thick Barium swallow under fluoroscopy control to actually see the swallowing movement on the screen and correlate with your symptoms can help to decide the cause and plan the treatment and whether the surgery would help you alleviate the symptoms or not.

Wheeze as we discussed above, needs clinical evaluation and actual examination with auscultation of the neck and the chest to get the correlation between your symptoms of wheezing and findings.

Will see the images once I receive them.
Certainly a Second opinion of a Radiologist can help.

I just don't really know which surgeons are good and that's what I am trying to search..

about wheezing my doctor thinks there are hyper secretions. But I wished we knew where they come from exactly if that's the case

Your Family Physician or the ENT can help you to get the correct Surgeon who can deal with your diverticulum.

Wheezing: your Doctor thinks about hyper-secretions. This will be obviously from the mucosa of respiratory epithelium or pharynx. Can be in response to chronic inflammation and when the area feel that there is something which is not alright. This is a reflex mechanism that the body has and works in a way to indicate that there is some problem and guides us to the problem to be found.

You can put the links or so of the Radiological findings as a separate query on this website asking for the Radiologist's opinion. Well, the things are already reported by your Radiologists too, the ones who have done the scans and we have to believe in them. Second opinion helps in cases when there is a doubt or a ? mark.

Yes, but it doesn't seem to improve. I am unsure. And I wished you could let me know which tests can we order to ensure they can find what's wrong as I assume the diverticulum might not be causing that. Shouldn't they be 2 separate problems?
