What Causes Vertigo, Dizziness And Itchy Right Ear?

default
Posted on Tue, 10 Jun 2014
Question: Four years ago is when this all started. Summer of 2010 I experienced extreme vertigo for two days. I thought it's got to be an ear infection, so I used the stuff for swimmers ear and it seemed to work. Fast forward 2011 same thing happens, except not as severe. come 2012, while driving down the interstate the whole world begins spinning. I pull over and decide it's time to go to the emergency room. I make the hhour drive and they pump me full of fluids and tell me I'm exhausted and dehydrated. In 2012 same things start happening even though I'm practically drowning myself with water drinking over a gallon a day. I go see a doctor, and she tells me that I have a minor ear infection and may be suffering from seasonal allergies. The dizziness stays until I move out of state in 2013. I go see another doctor, who tells me that it can't be an ear infection because it doesn't look like it. I get prescribed Ciprodex. For the first time in over 8 months I get to function without feeling dizzy. Now this year the dizziness has come back along with the itchy ear right before I have a vertigo spell. I tried Ciprodex at the beginning of spring and it seemed to clear it up, but it came back 2 months later. I have been to see yet another doctor who tells me I have allergies and he also told me to begin another round of Ciprodex, so I've done that and everything is worse. I've lost partial hearing in the effected ear and I'm constantly feeling as though my head is not attached to my body with minor dizziness. He also prescribed a round of antibiotics, a nasal spray which made me sick immediately, and some asthma pills. Mind you I have NO other symptoms except for vertigo and my left ear occasionally hurting, but mostly itching right before I have a dizzy spell.
I cannot tell you how upsetting this all is, especially when I've seen 5 different doctors who have had various different opinions and no one seems to want to listen when I tell them I need to see an ENT because clearly something is wrong that they aren't finding. Please help. All these meds are making things much worse.

Hi,

Thank you for your query.

1. Middle ear pressure changes or fluid build-up due to a seasonal allergy and sinusitis may cause such symptoms.

2. Get an Impedance Audiometry (Tympanogram) and a Sinus + HR (High Resolution) Temporal Bone CT Scan. In addition you will need to get a detailed ear and nose examination including otoscopy and a nasal endoscopy. The vertigo history and test for balance are required under direct observation. What is your age?

3. If you can upload the detailed reports and clinical examination findings, I will be able to suggest further treatment.

I hope that I have answered your query. If you have any further questions, I will be available to answer them.

Regards.

Thank you Dr. Bhatti.

I am 34.

The most recent examination states:
serous otitis media of both ears
otitis externa
Non-allergic rhinitis
Food allergy

WE did an allergy test and it turns out I do Not have food allergies, but I am allergic to bermuda grass, XXXXXXX grass, pigweed, elm tree, white ash tree.
I have no symptoms in my right ear, only left. No sneezing or stuffy nose, no coughing or wheezing, ONLY vertigo and left ear itchiness before vertigo and occasional pain. The pain has only started since this round of Ciprodex though.

Of which only bermuda grass is prevalent here in XXXXXXX TX right now according to the doctor. He has since referred me to an ENT, but it may take weeks to get an appointment. I'm kind of stuck between a rock and a hard place. I'm on the last day of the Ciprodex, which is good because it seems to make things worse not better, and now my ocipital gland on my right side is swollen which happened overnight 3 days ago.

I have no history of ear infection as a child or adult, and this is the first I've heard of me having seasonal allergies. I've never had symptoms of seasonal allergies now or historically. I had full blood work done last year and everything besides my Vit D was normal, some panels even better than normal.

I just want to know if this is something I need to really push to get seen about with this ENT or should I just calm down and wait. Unfortunately I have to leave to take my son to summer camp the beginning of July and will be gone for two months, unable to see an ENT here. Is this something I need to have taken care of immediately or something that can wait another year? I think my sanity won't survive this and certainly driving like this is dangerous, so I'm erring on the side of trying to figure out how to get seen before I leave. I just don't want to be a pain in the butt unless I need to be.

Hi,

Thank you for writing back.

1. If you have a normal Impdeance Audiogram / Tympanogram, you do not need any surgical treatment. Medication will be enough.

2. There is no cure for allergy, however with medication, it may be controlled. A non-sedating anti-histamine and a leukotriene will help.

3. A trial with a different nasal steroid sprays will help. These sprays require a week ti ten days of regular use to show their effects.

I hope that I have answered your query. If you have any further questions, I will be available to answer them.

Regards.
