Brief Answer:
Most likely to be due to Urethral Stenosis.

Detailed Answer:
Hello
Thanks for your query ,based on the facts that you have posted it appears that you have recurrent episodes of UTI and hematuria since 4 months and have been taking antibiotics.

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Have you ever consulted qualified Urologist
2) Have you done urine culture and taking antibiotics as per culture report

The most likely cause for your symptoms is due to what is called as Urethra Stenosis which is very common in elderly females due to generalized atrophy of genito urinary organs .

Consult qualified Urologist for assessment and get endoscopic examination and calibration of urethra done .

Self intermittent dilatation at home will help to resolve the problem .

Hope I have answered your query ,please feel free to ask if you have more questions , I shall be happy to help you .
Thanks and Regards.
