Brief Answer:
May be related to heart or induced by the blood pressure.

Detailed Answer:
Thanks for the consultation at my virtual clinic HCM!
I have carefully worked through your case, and can well realize your health worries. Being your physician, I will take care all of your medical concerns today and will put my best efforts to get you out of this situation.

Probabilities:
It usually is seen with longer hours of sitting or standing.
Hypertension is one of the major causes.
Some medications e.g. Amlodipine also causes swelling of lower extremities.
Kidney failure may cause the issue.
Liver failure is one of the reasons.
Lymphatic obstruction may cause the issue.
Incompetent valves of the leg veins are also the cause in some.

Bilateral lower extremity edema is usually caused by the excess fluid accumulation that has very complex mechanism and pathophysiology. You don't need to go into those details.

What you have to do is go to an Internal medicine Specialist for complete evaluation that may include but not limited to:
Thorough physical examination.
All baseline diagnostics.

Treatment goes according to the cause after reaching to an appropriate diagnosis.

Avoid long time standing and sitting. At night keep pillows beneath the legs to raise them above heart levels. Remain physically active. Avoid high protein diets. Take vegetables, fruits and salads.

Hope I answered all of your questions. Let me know if you have further clinical concerns or doubts. I will be more than happy to get back to you with expert opinion. Please don't forget to give Five Star rating to help me assist my patients in a better way; and, please don't forget to 'Close This Discussion'.
For any disease you have, a timely follow-up is very necessary regarding the treatment efficacy and to see if there is a need to change the treatment options. So keep a follow up to see how you doing (Always write down my name in the start of your questions at HCM & I will be with you in no time).
