What Causes Stomach Pain And Cramping In Lower Pelvis Area During Ovulation Time?

Question: I want Dr. XXXXXXX XXXXXXX M.B.B.S., MS OB/GYN, FMAS+DMAS to answer my question:
In continuation of our discussion I have some question to Dr. XXXXXXX
1. I don't know is it normal or not. During or before ovulation time (D16) I am feeling a much pain or cramping in lower pelvis area. I could not tell you properly the pain or cramp is like menstruation pain or due to acid problem. I have also constipation problem. May be due to constipation am I feeling pain?
2. This type of pain/cramping I have been feeling for last few years but not all months. This pain remains generally 4-5 days. I came to know this type of problem may be due to Endometria issue. But if you see my attached USG, my endometrial is ok.
3. Is the problem due to cyst? But my USG report says nothing. You can see may CT scan report also.
4. If you see my doctor prescription, Duphaston 50 will start from D16. During this time if I take TAB Bisacodyl 10mg as recommended by you to rectify constipation, is any problem?

Thanks for the follow up.
Answers:-
1. The pain and abdominal cramping that you are experiencing is due to ovulation when the XXXXXXX follicle ruptures and this is also known as MITTELSCHMERTZ and can last or 2-3 days in the mid cycle indicating ovulation. And this is unlikely to be due to constipation per se. You may try hot fomentation locally twice a day along with TAB MEFTAL SPAS 500mg twice a day for 3 days to relieve that pain. Its an antispasmodic.
2. Your ultrasound pelvis is normal and no evidence of endometriosis at all so this pain is not because of endometriosis, which occurs during menses when endometrial shedding occurs not in the mid cycle.

3. This is not a problem but a natural phenomenon and is not due to cyst.The cyst is a corpus luteum cyst which is often formed after rupture of XXXXXXX follicle and release of ovum. This may resolve by next cycle.

4. You can start duphaston from day 16 and if you feel you are constipated then single dose of BISACODYL can be taken but your water intake should be high as this is an osmotic laxative which shall help you relieve constipation.

Hello,
This pain is not experienced by all but but many and is a known fact so one needs to bear it and can be symptomatically managed with NSAIDS.
The pain usually lasts for 2-3 days and then subsides naturally.
So be patient.

BISACODYL 10mg is oral tablet. right? and can be taken at bedtime. Right Doc?

Yes bisacodyl has to be taken orally 10mg and just before sleeping and take 2 glasses of water with it. Next morning your motions should be smooth.
