Brief Answer:
Needs investigations.

Detailed Answer:
Hello,
Thanks for trusting us with your health concern.
The menstrual like cramps and spotting could be due to the effect of HRT. In case the spotting is persistent then a uterine pathology like endometrial hyperplasia or polyps need to be ruled out. Any cervical lesion or vaginal infection also needs to be ruled out.
You need to consult your gynecologist for ultrasound scan, endometrial biopsy, papa smear and vaginal swab test to find out the exact cause.
Please feel free to discuss further. I will be glad to answer the follow up queries that you have.
Wishing you good health.
Regards.
