Question: Sore scratchy throat, hard to breath, no flem, raspy voice, the 3rd week already, persistent cough, mucus is clear.

Hi Dear !! Thanks For your query to HCM.
Read and reviewed facts of your query in context to to your persistent cough for 3rd week.
My Impression-
In view of past history for last 7 yrs, the current cough seems to be non relieving for last 3 weeks.
I would suggest you to have Sputum AFB / Sputum Gm stain.

Remedy-
Plenty of fluids
Saline with Listerin Gargles x 5 days
Cap Augmentin (Amoxycilin with Clavulanate)625 mgm 1 x 3 times a day x 10 days time.

-If no relief with this course of treatment-
Then Investigate to rule out TB as the cause of this 3 weeks cough.
After review of your Medical history,the current breathlessness with scratchy sore throat seems to be extended for 3 weeks.
and hence check up for Sputum AFB and Sputum Gm stains is to be done to rule out the possibility of TB.
AS your sputum is clear, Sputum with PCR test for AFB Myc.Tuberculosis would be helpful to confirm unnoticed TB.
IGRA(Immune Gamma Release Test) blood Test is done to rule out hidden and latent Tuberculosis.
X-ray Chest PA to study for PTB(Pulmonary Tuberculosis)

-Other reasons to be ruled are Eosinophilia with Bronchospasm with Bronchitis and cough.
CBC- Complete Blood Test- for Eosinophilia.

Before treating this case for Pulmonary Tuberculosis, I would investigate and treat your case for Tropical Eosinophilia
Test for Stool Ova / Cyst for ascariasis
Blood for Eosinophilia
Tab Mebex -1 x 2 times per day x 3 days time.
Tab Antihistaminic -Dayquil with D+
Tab Anti-inflammatory +
Tab Hetrazan fort 100 mg x 2 times a day x 21 days
Tab-Bronchodialators- like -Salbutamol with theophylline to control irritative cough.

-If no relief with this, then I would plan to investigate your case for Pulmonary Tuberculosis,as the cough is more than 2 weeks with breathlessness,which indicates bronchitis.
Thus if the Antibiotic / bronchodialator treatment doesn't relieve the cough
then I would try for treatment for Tropical Eosinophilia , and still if cough persists,
Then I would investigate for latent TB infection as suggested above and treat it accordingly after confirming it by various tests as discussed above.
Good Supplementary diet / Rest / Plenty of fluids are a must for a long remissions from such a cough.

Your cough is a repeating story for last 7 yrs.So it appears to be Seasonal Asthma with Bronchitis.But tropical eosinophilia and pulmonary tuberculosis needs to be ruled out, one after another after drug trials as suggested above.
laryngopharyngitis could be of seasonal Allergic origin/ or could be from Tuberculosis / or could be from accompanied Tropical Eosinophilia with ascariasis.
