What Causes Sharp Right-sided Abdominal Pain And Vaginal Bleeding?

Question: Since Saturday night I have been experiencing a sharp pain on my right side. I had my tubes tied 3 month ago right after a successful vaginal delivery. A day after the pain began I noticed a blot clot and been having a menstrual type bleeding. The only thing is that I just had my period at the beginning of March and had been bleeding heavy for almost 3 weeks.

So let me summarize:
1. I do not know your age, I will assume in your 20-30's
2. You had a vaginal birth 2-3 months ago
3. You had a tubal ligation right after this birth
4. Now, you had the sudden onset of right sided pain and vaginal bleeding
5. Your last period was in early March
6. You had heavy bleeding for three weeks (??leading up to your period)

First of all, never assume that pregnancy might not be an issue. You need to check a pregnancy test to rule this out. I will assume that this is negative. In this case, especially if you are breastfeeding, your cycles are understandably irregular given how close you are to a vaginal delivery. Sometimes when your cycles are irregular, it is because you are not ovulating regularly and this can lead to a buildup of the uterine lining that will then shed irregularly. The pain is most likely related to an ovarian cyst, but given the location, there is also the consideration of appendicitis. You need to do the following:

1. You need to be seen
2. If the pain is severe, and if you have any loss of appetite or nausea, you need to be seen now
3. You need an ultrasound of the pelvis to evaluate the right ovary AND the uterus
4. While retained tissue from the delivery is extremely unlikely, this is a possibility
