I read your question and i understand your concern. Severe pain of acute history of a day radiating from back to shoulders is likely a musculoskeletal pain . Age related musculoskeletal pains and spasms are very common and need symptomatic care with NSAIDs, rest and some muscle relaxants like Skelaxin etc.
Take some NSAIDs, some muscle relaxants and some antacids as well as some times its gastric pain that presents on the back. Rest for a while and if the pain persists despite 72 hours of medications seek a doctor for further management.

HE IS ALERGIC TO NSAIDS! HIS BLOOD PREASURE IS 188/109/78! HE AS TAKEN A MUSCLE RELAXER AND A 5 MG OF OXYCODONE! STILL BAD PAID. IT MOVES FROM THE MIDLE BACK AROUND THIS SIDE TOWARDS THE FROM ABOUT 5 INCHES BELOW THE ARM PIT.

WIth history of blood pressure now and still having high readings i would suggest an urgent visit to ER and rule out any cardiac pain and decrease the blood pressure and maintain it as that pain can present any where around the trunk atypically. Once that is ruled out the pain will be managed with thousands of possibilities other than NSAIDs but first things first. A good start will be an EKG followed by cardiac perfusion study like Echo.
