What Causes Severe Pain In Thigh And Knee Of Right Leg With History Of DVT's?

Posted on Sat, 14 May 2016
Question: Good morning.

I'm wondering if its possible to have another blood clot in my right leg. I have had 3 so far since may 2015 and am currently taking 20mg of Xarelto every morning. I have not missed a dose in over a month.

I'm experiencing gnawing pain in my thigh and knee of the right leg which is the side all of my clots have been diagnosed previously. there is no temperature difference but there is some swelling of the affected leg.

I underwent hardware removal surgery on February 1 of this year. I have been in an aircast when I venture outside for safety reasons however I'm completely XXXXXXX since 48hrs after surgery. I did start physiotherapy February 19th 3 times a week so I definitely could be experiencing the symptoms from that.

I was also wondering if its OK to use Voltaren emugel cream to rub on my thigh and knee as the pain at times is enough to interfere with walking and activity.

thanks.

Brief Answer:
Please help me with some more details before I can give complete advice.

Detailed Answer:
Hi,
There are some questions if you could help me with them.
1. Did you suffer DVT?
2. Were the clots in the veins of one leg or both?
3. When was the first time it happened and what all veins were involved? If you could send me the Color Doppler report it would be helpful.
4. What surgery did you undergo? What hardware was removed? Did you have an IVC filter placed?
I would know about your condition better if you could help me with answers to these questions.
The treatment with Xarelto is fine for DVT.
Massaging with gel is not advised as long as there are active clots in the veins as then there is fear of the clots dislodging to the lungs.
Please do let me know the details so I can advice you completely.
Take care,

Hi again.

Yes i had 3 diagnosed dvt's.....first one on may 31,2015 in a single vein behind my right knee. The second 2 were discovered November 3rd 2015. There were 2 separate clots in both the main veins behind my right knee.

I've only had DVT issues with my right leg.

I had 2 screws placed in my right foot to fix a lisfranc injury on April 8th of 2015. I had 5 fractures as well as apparently the area was very unstable and the ligament was in danger of being unfixable. The screws had to be removed February 1st 2016 due to extreme pain and one of them had come loose and was wiggling around.

The first DVT was directly related to the first surgery in April but the 2nd 2 had nothing to do with the injury or surgery I was told. I am waiting for blood tests to come back to see if I may have a genetic disorder that predisposes me to repeated clots.

I don't know what an ivc filter is and have never heard of it so I'm assuming no I didn't have one placed. I don't have the doppler scan but I can attempt to ask the hospital if you need it

Brief Answer:
Thanks. The treatment is fine. Doppler report would help.

Detailed Answer:
Hi. Good to hear from you.
Thanks for all the information.
It seems you have leg vein DVT and most likely your thigh veins are spared.
The tests for genetic thrombophilic state would be good.
The line of treatment is correct. But you need to take Xarelto for 3 to 5 years.
Also, you need to wear Compression stockings specially while walking or during exercise.
And keep the limb elevated while lying down.
Take care and do let me know the Doppler reportif you can get it. That would give a great deal of information about the condition.
Take care and do let me know.
