What Causes Severe Cough And Neck Discomfort While On Ramipril?

Question: I am a Type I diabetic and have been on ramipril for years but developed a severe cough and neck discomfort. Have been taken off this med and was given Losartan 25 mg. to take and had severe low BS reactions while on this med.
Now wondering since Atacand and Avapro are in the same class, why does it not talk of side effects of
low BS reactions.
I have had diabetes for 55 years, and since being off BP med. I have had to have laser eye tx's. on both eyes. My opthalm. suggest I get back on a BP med. but questioning which one of these or any other BP might work for me.
I have hypoglycemia unawareness but my kidneys and overall labs are good. Now the doctor has given me all the information to read on these drugs, but can't understand how they- meaning Losartan and the Candesartan and Irbesartan drug on in the same family but is one better for not producing low BS levels. I also wear a CGMS. and on an insulin pump.
Thanks for any input you might have. I just want to protect my kidneys and eyes and do not have hypertension. Age 62.
Eye doc. stated my blood vessels are very small in my eyes and did have a vessel break a few months ago.

There are different types of receptors in the body. Receptors are points that respond to certain chemicals.

There is a receptor called ppar gamma. This receptor plays a part in insulin sensitivity ( how the body responds to insulin).

SOme medications can affect this receptor, therefore increasing insulin sensitivity.

Losartan is one of the medications that may affect this receptor.

Avapro ( ibesartan ) is not thought to have an effect. There is conflicting information about candesartan. Some medical articles state that it may, others do not.

Though the medications are in the same family , slight differencs in their structure and mechanism of action cause differences.

I noticed that Ibesartan is excreted mainly through the bowels.
Could this med,. Avapro, possibly build up in ones system if they have a slow digestion system? To be specific, I seldom have daily BM's and attempt to make sure I have one every three days.

We have agreed that I would be put on a very low dosage since I am very sensitive
to insulin and really do not have high BP but attempting to protect my eyes, kidneys and heart!
Is either of the meds we have discussed a problem w/ Mitral Value Prolapse.
Also, have read precautions in ref. to mitral stenosis??

HI

Sorry about the late reply

It is broken down in the liver by liver enzyme system CYP2C9 and CYP3A4. Its metabolite (what it is broken down into) is an inactive substance calledirbesartan glucuronide.

This is eliminated via the bowel and urine , about 20% urinary.Because the metabolite is inactive, it would not matter if the transit time via the bowel is slow.

There are no known precautions with mitral stenosis
