What Causes Severe Chest Pain After Weight Lifting?

Question: Dear Doctor,

I am 35 years old male. Recently, I had chest pain and it came to me after I lifted heavy weight. The pain are is above the upper chest and below the shoulders.
I get pain in the right chest & very rarely on the left, if I walk continuously or lift heavy weight or walk staircase or drive my bike.
The pain was unbearable and I decided to take ECG & ECHO. Echo came normal, while ecg showed some changes with sinus tachycardia.
The technician ordered me to come after two days, since I was very anxieted and stressed. Even on the second day, the ecg levels were not normal. So he asked me to perform Angiogram (assuming it to be a disturbance in the blood flow). It was a shock to me, since I had acquired this pain only after lifting heavy weight).
I did not go to Angio, I waited for a week and yesterday I performed the following tests on my own (with the help of internet information). The results are as below:
1.     CRP (C-Reactive Protein) – 3.0 mg/l
2.     Cholesterol Total – 260
3.     TGL – 190
4.     LDL – 180
5.     Troponin T – Negative
6.     Creatinine Kinase Total – 139
7.     ESR – 9
8.     Hemoglobin – 14.5
In addition, the Urea, creatine & sugar levels were normal.
Is the cholesterol level so high for my age? Do I need to really take medications or is it enough, if I had to change my lifestyle & diets?. Please suggest me some ideas to reduce my cholesterol levels. Please, I do not want to start with medicines. Please.
I am planning to see an Ortho doctor for my pain. Please kindly advise.

I can understand your concern.
I have gone through the reports you have attached.
Yes, your cholesterol levels are high for your age.
Total cholesterol should be less than 200.
LDL cholesterol (main culprit to cause heart attack) should be less than 100.
And at young age (active age), cholesterol should never be above normal levels.
So you should definitely start treatment for this.
Before commenting further about treatment, diet and exercise, I want to know
1. Ecg (please send me pictures of your ecg)
2. Your height and weight.
2d echo can be normal even if you are having blockage.
Ecg is the screening test for heart attack.
Next step should be TMT (trade mill test).
If this is normal then no need to go for angiography.
And one should not do weight reduction exercise if he or she is having acute ecg changes.
So first send me pictures of your ecg.

I can understand your concern.
I have gone through the other ecg.
Both ecgs have minor changes.
In my opinion, you should get done TMT first. No need for angiography directly.
Get done TMT, if this is normal then no need for angiography.
You should also start Rosuvastatin 20mg for cholesterol.
Along with Rosuvastatin, strict dieting, exercise, yoga etc will help in reducing weight and cholesterol levels.
