Question: Hi

U was diagnosed with candida after presenting a red and burning penis head. At the same time I also had a burning tounge and lips with the tounge being white in colour.

These symptoms continued for 1 month of treatment with 1% topical cream. I was the tested for all STDs (including HSV via ureathral swab) but all were negative. U was prescribed flucan tablets, 50mg per day for 1 week and tmthey sympims appeared to clear up but within a week they were back and have been for a further 2 months (4 months in total)

Symptoms are generally red penis gland which looks like its aged and can burn or be a bit uncomfortable. It can sometimes appears to shine in the morning.

At a loss and really want to know what next steps to take.

Brief Answer:
I suggest screening for blood sugar.

Detailed Answer:
Hello. Thank you for writing to us.

I have gone through your query and I have understood your concern.

Raised blood sugar is a very common cause of recurrent candidal balanitis and therefore I suggest that you get screened for fasting blood sugar levels.

As far as treatment is concerned you must restart oral and topical antifungal treatment. However I would suggest a different schedule i.e single tablet of fluconazole 150 mg every alternate days for a week(total 3 doses), along with a topical antifungal e.g clotrimazole 1% cream preferably the one which is in combination with beclomethasone dipropionate 0.025%, twice daily.

Avoid sexual contact for a week. Sexual partner should also be similarly treated if she complains of vaginal discharge or vulal itching.

Thank you for responding

I will take the fluconazol as suggested.

However, my main concern is that this could still be Herpes. Do you have any views in this and whether I should get a specific blood test?

Kind regards

Brief Answer:
Genital herpes usually presents with small erosions

Detailed Answer:
Hi.

Genital herpes usually presents with small (2-3mm), painful, genital erosions on glans or genital skin.
If desired you may take a blood test for Hsv i.e ELISA for Hsv type 1&2 (IgG and IgM).
