What Causes Red Spots On Penis?

Question: I have a red spot on my penis. It does not itch or change in any way. It has little or no feeling. Has been their for several months. The Doctor on this list suggested I talk to a specialist. I uploaded a pic for him. I do not know if you can see the pic. If not let me know and I will send another.

Hi.

I have reviewed the Image.
I can see a single well defined glistening orange red patch on the surface of glans penis and adjacent prepuce.
I suggest a possibility of Zoon's Balanitis/ Plasma cell balanitis. It is almost always seen in uncircumcised middle aged men and is never to be seen in circumcised men.
Confirmation of diagnosis by by a tissue biopsy which demonstrates copious amount of plasma cells infiltrating the upper dermis
Topical steroids and topical tacrolimus ointment can be tried, however, the condition frequently recurs.
Only Circumcision if curative.
I suggest you to visit a dermatologist in your region for a confirmatory diagnosis as well as the needful.
Your dermatologist might refer you to a general surgeon for circumcision as deemed necessary.

I have been circumcised. I was as a baby. I had the condition a few years ago and it went away. Is the treatment over the counter drugs? If not can you give me a perscription and lets see how it works.

Hi.

Thank you for letting me know that you have been circumcised since childhood.

Zoon's balanitis is as such commonly seen only in uncircumcised middle aged males and circumcision is rather curative in Zoon's.

That does lead me to keep another possibility of erythroplasia of queyart/ bowen's disease, which is actually squamous cell carcinoma in situ.

Bowen's disease may similarly present with asymptomatic, well defined, red patch on glans
"In situ" means that carcinomatous changes are localized to the superficial epidermis and it has not yet disseminated.

I would suggest a tissue biopsy for a confirmatory diagnosis.

My clinical differential diagnosis would be both erythroplasia of queyart/ bowen's disease and Zoon's balanitis.
Tissue biopsy would be confirmatory and would guide subsequent treatment.

If it does reveal bowen's/ carcinoma in situ, the lesion can be simply freezed with liquid nitrogen.
Otherwise, if the tissue biopsy does not reveal carcinomatous changes, topical potent steroids+antifungal combination creams can be tried.
