What Causes Recurrent Fever In Children Despite A Normal Blood Report?

Question: My name is XXXXX... and I wish to ask you a question about my son.. he is 3 years old and on 26 Sep he got fever and then I went to doctor and he prescribed cefixime 100 mg bd.. and my son got well with in 3 days... but yesterday night again he got 102 fever and I gave him paracetamol.. and in the morning doctor suggested us cbc. Malaria and urine rm test... coz he got some urine infections in past months.. I cannot understand all things. But after Seeing reports I know he has no malaria and he has no urine infection.. but I am not able to understand blood report.. that is why I am asking you..

What problem does my son have?

Coz his hb is little bit low.. but I think tlc is too low. It should be 5000- 15000 but it is 3900 cmm.. I am worried about my son.. and I do not know why he gets ill again and again... and plz tell me how to increase tlc... I will show this report to my doctor tomorrow in the morning.. but before that I can't wait.. so I want second opinion from you.. and I wish to tell you that he prescribed my son ofloxacin 100 mg syrup 4 ml bd . Before seeing report... so plz answer my questions and help me out.. 3900 TLC is too dangerous or it will be ok by antibiotics...

Hi
I have gone through your question and the reports. Do not worry. The most likely reason as per symptoms and reports seems to be typhoid. It can be easily managed with proper course of antibiotics.

At present, I would recommend you to go for his blood culture and sensitivity test. The reports take 3-5 days. So, start him on Syp. Cefixime (15-20mg/kg/day in 2 divided doses) after giving blood samples. Continue paracetamol for fever spikes if needed.

Upload the report of culture after it comes.
Do not worry about TLC. These are protective cells in badge which improve once the disease is treated. Also, give iron rich food for mild anemia.

thank you mam for your answer.. and I called my doctor for showing reports.. and he told me to come with reports today.. and i went in the evening after seeing reports he also told me about typhoid.. and when I asked him for any test he refused me.. and he prescribed azithromycin 200 mg syrup 3 ml daily it means 120 mg .. my son's weight is 12.5 kg.. and he also told me for taking ofloxacin 100 mg syrup 4 ml dose twice a day it means 80 mg two times.. and he told me to come after 5 days.. and he also told me if fever is there take paracetamol 200 mg SOS ...

mam I wish to ask some things.. so plz give me answers..

1. can we take paracetamol again n again and after how many hours paracetamol can be taken.?

2. plz tell me mam the condition of my son. coz my doctor is not telling me that how severe this typhoid is.. it means what is the stage of this typhoid? is it starting or middle stage? plz mam give me my this answer plz.. suppose there are 5 stages and 5 th one is too dangerous. so what is the stage of my son's typhoid..? coz i am worried..

3. please also tell me about iron syrup.. because my doctor told me for stopping iron syrup.. he was saying to me that antibiotics will not work with it.. so please tell me when can I start iron syrup?

4. and I would like to also know that somebody told me Typhoid fever is recurrent fever and it comes back again and again is it right after completing the full course of antibiotics my son will be ok and it will not come back again and again.... plz mam give me all answers.. coz I am in tension....

Hi
The report is suggestive of typhoid but it needs confirmation by proper blood culture and sensitivity test. So, I would recommend you to go for culture first as suggested earlier.
Start with antibiotics I.e. cefixime as it is the first line drug and was given for just 3-4 days earlier. Add azithromycin if fever doesn't improve in 3 days. No need for ofloxacin till blood culture report comes.

Paracetamol dose is fine. It can be given every 6-8 hours if needed. Don't worry as it is a safe drug. Most kids improve and become afebrile in 3-4 days.

There is no such staging for typhoid. As such your child seems to have just uncomplicated typhoid at present and will hopefully improve with proper line of management. Please do not avoid blood culture. Typhoid is not recurrent if we'll treated with full course of antibiotics.
