Brief Answer:
Most probably chronic urticaria ....

Detailed Answer:
Hi,

Welcome to HCM...

After going through the history you provided, I could conclude that most probably you would have chronic urticaria. IgE and AEC are elevated. And the wheals you have repeatedly is more than one and half month..right? This means that most probably you suffer from chronic urticaria.Chronic urticaria (CU) has a wide spectrum of clinical presentations and causes.Wheals may occur almost daily in some individuals or it may occur periodically in others.

In many patients, in spite of extensive investigations, the cause remains undetermined. So, some times, it may be called idiopathic chronic urticaria. Now it is known that autoimmunity is the cause of CU in 50% of cases. And thyroid autoimmunity is often associated with the condition. Dietary pseudoallergens may aggravate existing CU.

I strongly recommend you to consult the dermatologist. And have thorough investigations. There are number of aggravating factors that can be avoided by simple measures. Your doctor can identify the same with careful history taking. These include diet, drugs, alcohol, viral infections, local heat and friction, and mental stress.

Many times, diet is often considered as a cause of urticaria. Now we know that pseudoallergens may be important cause in some patients. Pseudoallergic reactions to additives, natural salicylates and aromatic compounds are known.

Certain systemic diseases and infections may occasionally lead to urticaria. So, systemic diseases should be ruled out by complete blood tests.Rarely, common environmental provocations such as sunlight, water, cold, heat, exercise, and pressure seem to induce urticaria.

However, there are various modalities of treatment.....

Antihistaminics,steroids,doxepin,methotraxate,cyclosporine,mycophenolate mofetil,intravenous Immunoglobulin,plasmapheresis,autologous serum therapy...etc

Kindly strictly follow your doctor's advice.Have patience for the good result.

Eye problem you have might be unrelated to your skin problem,so consult ophthalmologist.

You have pain in right knee, may be due to osteoporotic changes, please consult orthopedics.

I hope this would help you in managing your skin disease. Still, any query..you are welcome.

Thanks.
