Question: Hello im 31 year old woman and i am having some dull achey pain in my back that radiates through to the front. Its on my left side and i have shortness of breath, dizziness and nausea. I dont have insurance so i dont want to go to the er unless i know i need to.

Does the pain change during body movements or respiration? This would indicate a musculoskeletal pain.

Have you suffered before from any similar symptoms?

How long have these symptoms been going on?

If your blood pressure is unstable and if this situation persists, I would recommend going to the ER for a careful physical exam and some tests:

- a resting ECG and cardiac enzymes
- a chest X ray study
- some blood lab tests (complete blood count, blood electrolytes, kidney and liver function tests).

Hello thank you for answering so quickly. The discomfort in my chest doesnt change with movement but i feel a lot more dizzy and nauseous. I had something similar happen after i gave birth to my son 5 years ago they did an ekg and said i had an irregular heart beat but i wasnt told to follow up about it, but heart diseseruns in my family. I woke up feeling achey but it has progressed as the day has gone on. I also feel very tierd and like im going to black out. Hope all this helps.

I would like to have some more information on your blood pressure values, because some of your symptoms (dizziness, nausea, tiredness, shortness of breath) could be also triggered by high or low blood pressure.

Regarding your chest pain, it is necessary a differential diagnosis.

For this reason, if your chest pain persists and also to clarify your actual heart rhythm (to investigate possible arrhythmia), I would recommend you to discuss with your doctor (or ER doctor).

But in case, your pain will disappear and the only obvious reason remains high blood pressure, probably you could avoid expenses by going to the ER and just try to control your blood pressure by modifying your daily lifestyle and diet.

I would also recommend considering gastro-esophageal reflux as a possible cause of this clinical scenario. I would advise you to try taking an antiacid or PPI (omeprazole, pantoprazole, esomeprazole, etc.). If the pain is relieved by these drugs it would be a strong argument in favor of this disorder. These drugs are safe to be taken in your clinical situation and you can safely make this trial at home.
