What Causes Puss Filled Blisters Around Buttock And Anus Region?

Question: About a 5 days ago I had an itch in my butt crack and I thought it was a pimple so I neglected it. About 3 days ago I saw a cluster of puss filled pimple like things on the back of my thigh (see pic), a day later I get the same cluster on my Anus crack one on top and one on the bottom. After it, i get the same clustered rash near the top of my testicles near the base of the penis. I also have small pimple like things on my penis now (see photo). All of these clusters are on the right side of my body, towards my right hand ( See pictures). I have a girlfriend for the past 2 years and we regularly mess around but dont have sex and nothing has ever happened in the past. She is clean and std free and so am I. Could this be herpes? I am extremely worried. My gf is a virgin and I am fairly confident she isnt the one responsible for this. I have recently started sweating and going to the gym a lot and i take showers there and wipe with their towels, could this be the reason? The clustered rash is painless but they do feel itchy and numb when i touch it. (See photos). I have no pain, no fever, no swollen lymph nodes, but I do have a mild pain to the right crotch area (groin) when i press and apply pressure. Please advise, i am really scared.

Brief Answer:
Consistent with herpetic rash

Detailed Answer:
Dear XXXXXXX

Thanks for your query and the good quality pictures. The sharp unilateral distribution, along with a "dew drop" appearance of some of the lesions as vesicles is more consistent with Herpes; the best diagnostic methods include a scraping, tincture (called Tzanck) and viral culture of the content of one of the lesions; serum antibodies for Herpes 1 and 2 can also be ordered, but, it is better to assess the lesion contents directly.

Hope this helps, wish you the best,

Thanks Doctor, my girlfriend has no signs of herpes and has never had anything like this. I have never experienced this before either so how can this be herpes? I would have gotten it from her, if not, I could be a carrier all along from past relationships but that would mean I would have given her herpes within this last two years? She has no signs of anything. Its confusing because it happened out of nowhere and I highly doubt my gf is a carrier of herpes. Is it safe to assume that she has given it to me?

Brief Answer:
Not safe to assume that

Detailed Answer:
It is tricky if this is herpes. It may mean:

1) Inoculation from oral Type 1 herpes (aka cold sore) during manipulation or oral encounters

2) Like you well said, sometimes there is an initial low-symptoms inoculation that may remain inactive for some time and then recur

3) There is more than meets the eye

Now, the interesting thing is that we do not all have the same susceptibility to the virus; some people just develop antibodies and have good immunity, others just manifest the infection and it can wax and wane.

Again, I would suggest those confirmatory tests now that there are active lesions.

Would it be safe to assume that IF it is herpes, it would be HS 1? I am saying this because my partner clearly does not have HS 2.5/

Brief Answer:
More likely

Detailed Answer:
Yes, if a source such as a cold sore is the initial source, HSV 1 is more likely.

Hope that helps, if no further queries you can grade accordingly and close session.
