Posted on Fri, 24 Jun 2016
Question: Hi I've had a headache for the past 7 days. It feels like a pressure around my temples and ears. The pressure is strong and is making my life very difficult. I went to the doctor twice one said it was tension headaches and the other said I could have a sinus infection, but I have no nasal discharge or fever or anything like that. I don't want to take antibiotics because I have bad reactions to antibiotics, but I need this pressure to stop please help.

Welcome to health care magic,

I have read your query and understand the concern.

But before further discussion, I would like to know,

Is your headache associated with nausea or vomiting or any other symptoms?
Which medications you have been given by your doctor?
Did you get any relief from any medications?
Was there any triggering factor for headache?

Kindly get back to me with these answers.I will give my opinion after that.

Some more information is that my headache is not associated with any nausea or vomiting or other symptoms besides a headache. My dr. has advised me to take Advil which has not helped. My second doctor has given me antibiotics for a sinus infection which I do not want to take and have not taken yet because I get negative side effects from antibiotics and do not think I have a sinus infection. The headache came on seven days ago after I came home from college. It has remained constant and feels like a constant pressure on my temples. I had pain in the back of the head and top of the head which has gotten better, but the pressure is still intense on my forehead, temples, and side of the head. Could I have a sinus infection without any nasal congestion? What kind of headache do you think I have and what can I do to stop this pressure that I feel? It feels like someone is pushing on my temples and forehead. Thanks.

Also, I am having pressure in my ears which is concerning. I am nervous pressure on the head could be something more serious but I am only 21.

Brief Answer:
Take alternative pain killer combinations.

Detailed Answer:
Hello,
Thanks for sharing more information.

From all these more facts that you shared here, it is more likely to be having tension type of headache and it is usually relieved by taking over the counter appropriate pain killers and at the same time eliminating the triggering or precipitating factors (if there is any like gastric problem, dehydration or heavy sunlight exposure, sleep deprivation, wrong or uncomfortable posture, eye strains, stress, long working hours in one position etc)

Tension headache may last for few hours to some days depending on triggering or precipitating factors as mentioned above. And it is very rarely serious, so no need to worry about it.

But I will not think about the possibility of sinus infection here as it is highly unlikely to be having sinus infection without nasal symptoms.Your ear pressure seems to be related to this tension headache.

Antibiotics would have not much role here,so I am NOT in favor of taking antibiotics without confirming sinus infection by physical examination.

Therefore, in my advice, you should take alternative pain killer combinations like Tab.Ultracet (acetaminophen and tramadol combination) with muscle relaxant like metaxalone (Brand name: Skelaxin) if you did not get relieved from Advil alone.

Anti-anxiety medication (ex. Recita forte) would also be helpful to reduce stress level and if you have not adequate sleep.

But you will need prescription from some local doctor or GP for muscle relaxant and anti-anxiety drugs.

Take antacid which suits you like Pantoprazole 40 mg twice daily if you have gastric trouble or stomach upset because of pain killers.

Take plenty of fluids (simple water or lemon water) with lots of fruit juices (apple juice and Sapota milkshake would be best) for next 2 to 3 days. It would also be important to maintain proper hydration and electrolyte balance which in turn will help to reduce headache.

Avoid oily/spicy/fatty food or even routine diet and take bland diet only for initial few days.

From all these symptomatic treatment,we may expect to see improvement within 2 to 3 days and if not then it would be better to see physician or Neuro-physician for further evaluation.

Hope this help and let me know if you have more questions.

Regards,

Dr.Sandip Kabra
