Brief Answer:
blood stained sputum is a symptom of TUBERCULOSIS

Detailed Answer:
welcome to healcare magic,
with the symptoms you say like greenish yellow phlegm and blood stained sputum it is most likely to be a TUBERCULOSIS. other symptoms include unusual loss of appetite and weight loss. TB is not always seen in xrays and CT scan.
i want you to to do early morning first sputum examination for consecutive 3 days
next you have to have Mantoux test
the third xray a digital one
Tuberculosis is a treatable condition only. there are good anti tuberculosis drug available now in the market and the only thing is that you have to regularly take your medications for 6 months continuously, have a protein rich diet regularly
please review with me will all the above tests mentioned.
hope this explains you in detail
have a follow up if you still have more queries
thank you

Dr. I had the skin test in 1994 and it came out positive and I was given medication for 6 months. I definitely hack a lot especially in the morning sometimes black colour during the day it can be very white too. I got lot of appetite, no fever or night sweat the only time I coughed up something or hack is in the morning for so many years could it be I was having active tb all these years ive been told heartburn allergies and dripping sinuses. Nowadays I have headache that last few minutes and go away. Should I go direct to the hospital or to my doctor. Thanks

Brief Answer:
You have to get investigated in a hospital

Detailed Answer:
welcome to healthcare magic,
Sputum with black coloured or sputum with blood stained yellowish coloured sputum with night sweats with loss of appetite and loss of weight is definitely tuberculosis symptom.as you said your skin test is positive it suggests that mantoux test is also positive which confirms the diagnosis in 1994.
In the hospital now you have to be investigated for early morning sputum three consecutive days positivity in one or two sample for tuberculosis another time confirms the diagnosis.
you have to do another mantoux test to know the present status
and the other one is a recent X-ray Chest PA view (digital)
as you have said that you have already taken anitubercular drugs for six months now this time it will be considered as a relapse and you have to take 4 drug regimen for 2 months that is isoniazid ,ethambutol, rifampicin, pyrazenamide and streptomycin the first line drugs
and 3 drug regimen for 6 months that is isoniasid ethambutol and pyrazenamide
and before discontinuing the drug you have to do ESR test.
so the best option for you now is to attend a hospital and they will guide you. if you have all the results of the above mentioned tests along with the recent digital xray you can review with me also. hope for your speedy recovery with regards

Does my condition require isolation in the hospital?

Answered by Dr. Amarnath (2 hours later)
Brief Answer:
It is only determined with the results

Detailed Answer:
welcome to healthcare magic,
first you have to be investigated with the result of the investigation only I can say you need isolation or not.
in my opinion I personally feel that you will most probably need isolation now till you finish your first line of drugs that is the first 2 months .during the continuation phase that is during the last six months of three drug regimen you will not be infective to others
any way first do the investigation as I said to you . with the results of the investigation only I can definitely say you need isolation or not.
hope this explains you in detail
