Question: I am 51 yo and had a bad delivery 25 years ago w episiotomy that popped open a week after birth. Dr. Refused to restitch - said it needed to heal from inside out. It never did. Tried estrogen cream but it didn't work. Have suffered painful intercourse for years despite copious amounts of lubricants. Last night, during intercourse, my episiotomy tore open internally - not at the edge - about an inch inside. The tear is at least 1/2 an inch long and bled for hours. Do I need to see my Ob-gyn immediately today, or another one if he is unavailable? I can't use Premarin anymore bc I take Tamoxifen.

I'm so sorry to hear all the suffering you have been through so far.
Do not apply premarin at all now as it will increase the blood flow to the area and hence more bleeding.
You need to visit emergency room as soon as possible to any other gynecologist for examination and resuturing in case needed and possible.
Also take tablet tranexemic acid which will help in controlling the bleeding to some extent.
