Posted on Mon, 29 Dec 2014
Question: I have pain in left upper adominal pain. Dr said it's reactive. Same pain last year revealed on cat scan that there was fluid and swelling around kidney and non cancerous lesion on my spleen. This happened after a fall. Why the pain again

Brief Answer:
Reactive pain is one which is probably in reaction to earlier trauma.

Detailed Answer:
Hi XXXXXX,
Thanks for writing in to us.

I have read through your query in detail.
Reactive is applied to a situation where the pain might be perceived in a greater intensity due to the existing changes or conditions in that area. It points to a greater appreciation of the pain due to previous injury and does not mean in the head. This is not a serious condition but rarely the pain can get severe. I would appreciate if you can attach a copy of the report to your query so that I can understand the situation you were in last year in greater detail.

It is possible that the swollen kidney and fluid or the non cancerous lesion in your spleen might have caused certain changes in the region which is presently causing pain in the same area. Reading through your earlier reports might give me a better idea of your situation and provide greater insight. I recommend an ultrasound scan if the pain is limiting you from going about doing your daily activities.

Waiting for updates from your end.

Regards,
Dr.Vivek

Not sure how I'll get report. All I have is the ultrasound saying spleen lesion non cancerous. I have appt with GI doctor in 6 weeks. Starting to have hep.c symptoms after 12 years. Do you think this pain could have anything to do with liver and other nearby organs . My family doc seems to think I can wait till then but I think she said said that because they are really swamped. Should I insist on referral for ultrasound? Dizziness and BP going up and down concerns me.

Brief Answer:
Getting ultrasound scan abdomen done is justified for you.

Detailed Answer:
Hi XXXXXXX,
Thanks for writing back with an update.

It is important to note that the spleen is on left side and liver is on the right. Therefore if you hurt your left kidney last year and the pain is in that region, then it is unlikely to be the liver.

The other significant point is that you have hepatitis C. Your GI doctor should send you for an ultrasound scan if you are having symptoms after 12 years as you have mentioned.

Either way getting an ultrasound scan is justified in your case and I wish you get that done earlier and then discuss with your GI doctor during your meeting.

The BP needs proper evaluation and recording after at least 30 minutes of physical and mental relaxation and then it will be known if you have problems with blood pressure or it is due to anxiety.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek
