What Causes Pain In Upper Left Quadrant While Lying Down?

Question: ULQ pain for weeks. Under rib, sometimes spasms.bending forward/lying down increases pressure/pain. fullness and nausea .I do have have accessory spleen.

To recapitulate: Female/37 - pain in upper left quadrant - for weeks - under rib - sometimes spasms - increase in pain/pressure on bending forward or lying down - there is fullness and nausea - have accessory spleen...
The most probable causes are:
- Costochondritis meaning inflammation of the lower rib due to its inflammation and can represent the way that you have described; there will be local tenderness, comes and goes, Fullness and nausea can be due to reflex.
- Another less likely cause can be colitis, associated gastritis or so.

This can be diagnosed by:
Local tenderness.
Other investigations either to confirm or rule out the possibilities like gastritis and colitis by upper GI endoscopy, Colonoscopy, ultrasonography to start with and further tests if required. Accessory spleen may not possibly cause such a problem.
There can be an associated stress and/or anxiety that instigate or enhance the problems.
