Brief Answer:
Balanitis, Phimosis , paraphimosis with adhesions.

Detailed Answer:
Thank you for asking at HCM.

I read your question and i understand your concern. Painful penis with suspected infection and inflammation not responding to antibiotics is likely a penile foreskin adhesion with concomitant infections like phimosis, paraphimosis and BXO balanitis xerotica obliterans.

I believe these issues are due to uncircumcised foreskin ( although you did not mention whether your are circumcised or not) but mostly these issues rise with such history. If any recent unprotected coitus then STD evaluation should be ruled out as they are already ruled out as per your statement.

Keep the penis at rest, don't copulate or masturbate for a while, maintain a good hygiene and consider therapeutic circumcision. Wear loose undergarments and avoid sweating down there.

I hope it helps. take good care of yourself and dont forget to close the discussion please.

Khan

Brief Answer:
Conservative treatment

Detailed Answer:
Thank you for getting back to me

With penis circumcised, odds of balanitis are more and phimosis and paraphimosis are less. Rest the management is conservative with antibiotics and its the same. Wait and see and precautionary measures as advised above should be addressed.

I hope it helps.
